Immediate dental implant placement after tooth extraction?
Hello, doctor.
I had a tooth extraction yesterday followed by immediate dental implant placement for my upper right wisdom tooth and the adjacent tooth, which were sutured.
I applied ice packs on the same day and had a normal diet about five hours later (soft foods and juice).
I didn't experience much pain that day and continued to use ice packs.
However, when I woke up the next morning, I noticed a dark red, clotted mass in my mouth, hanging where the extraction was done, about the size of a raisin.
Is this a blood clot or could it be tissue that hasn't healed properly? What would happen if this mass accidentally falls off?

In the above situation, if there is any bleeding from a blood clot after surgery, you should immediately return to the dentist for an examination.

More Info
After undergoing tooth extraction and immediate dental implant placement, it's not uncommon to have concerns about the healing process and what to expect in the days following the procedure. Based on your description, it sounds like you are experiencing a common occurrence known as a blood clot, which is a normal part of the healing process after an extraction.
Understanding Blood Clots After Tooth Extraction
When a tooth is extracted, the body naturally forms a blood clot in the socket to protect the underlying bone and nerves. This clot serves as a foundation for new tissue to grow and is crucial for proper healing. The appearance of a dark red or blackish mass in the extraction site, resembling a raisin, is likely the blood clot. It is essential for this clot to remain in place for optimal healing.
What Happens If the Blood Clot Falls Out?
If the blood clot dislodges or falls out prematurely, a condition known as dry socket (alveolar osteitis) can occur. Dry socket is characterized by severe pain and delayed healing because the bone is exposed to air, food, and fluids, which can lead to infection. Symptoms of dry socket may include:
- Severe pain that may radiate to the ear, eye, temple, or neck.
- Bad breath or an unpleasant taste in the mouth.
- Visible bone in the extraction site.
If you notice that the clot has fallen out and you begin to experience any of these symptoms, it is crucial to contact your dentist or oral surgeon immediately for evaluation and treatment.
Immediate Care and Recommendations
1. Avoid Disturbing the Area: Do not poke or touch the extraction site with your tongue or fingers. Avoid using straws, as the suction can dislodge the clot.
2. Maintain Oral Hygiene: While it’s essential to keep your mouth clean, be gentle around the extraction site. Rinse your mouth with warm salt water after 24 hours to help keep the area clean.
3. Pain Management: If you experience discomfort, over-the-counter pain relievers like ibuprofen or acetaminophen can help. Follow the dosage instructions on the label.
4. Diet: Stick to soft foods and avoid anything that requires chewing for the first few days. Foods like yogurt, applesauce, and smoothies are good options.
5. Follow-Up: Attend any scheduled follow-up appointments with your dentist to ensure proper healing and to address any concerns you may have.
Conclusion
In summary, the blackish-red mass you observed is likely a blood clot, which is essential for healing after tooth extraction. If it falls out, it can lead to complications such as dry socket, which requires prompt attention. Monitor your symptoms closely, maintain good oral hygiene, and do not hesitate to reach out to your dental care provider if you have any concerns. Proper care and attention during the healing process will help ensure a successful outcome for your dental implant.
