Post-hemorrhoid surgery?
Hello, Dr.
Ke.
I would like to ask you about my condition.
It has been a week since my hemorrhoidectomy, and I can only have a bowel movement while sitting in warm water, which is extremely painful.
Is this a normal phenomenon? How can I ensure that I completely empty my bowels in one go? I noticed that during my sitz bath today, I passed gas, and later saw a large blood clot and fresh blood in the water.
Is this normal? Additionally, after my sitz bath and bowel movement tonight, I noticed a swollen lump protruding near my anus.
What could this be? I am very afraid of the pain...
Thank you for your response, as the hospital where I had the surgery is quite far from my home.

After hemorrhoid surgery, patients often hesitate to have a bowel movement due to fear, which can worsen the healing of the wound.
It is recommended to take stool softeners and pain relievers, and to bravely attempt to have a bowel movement.
If pain occurs, a warm sitz bath can help alleviate discomfort.
It is common to experience occasional bleeding within the first week post-surgery; as long as there is no continuous heavy bleeding, it is not a concern.
As for the swelling near the anus, it is usually due to postoperative inflammation.
Regular warm sitz baths and maintaining good bowel habits will help reduce the swelling over time.

After undergoing hemorrhoid surgery, it is common for patients to experience a range of symptoms, including pain, bleeding, and difficulty with bowel movements. Understanding these symptoms can help you manage your recovery more effectively.
Firstly, experiencing pain after hemorrhoid surgery is quite normal. The surgical site is sensitive and can take time to heal. Pain during bowel movements, especially if you are only comfortable while sitting in warm water (sitz bath), is a common complaint. The warmth of the water can help soothe the area, but it’s important to ensure that you are not straining during bowel movements, as this can exacerbate pain and lead to complications.
Regarding the presence of blood in the water after a bowel movement, it is not uncommon to see some blood following surgery. This can occur due to the healing process, as the surgical site may still be fragile. However, if you notice a significant amount of blood or blood clots, it is essential to monitor the situation closely. Small amounts of blood can be expected, but large clots or continuous bleeding may indicate a complication that requires medical attention.
The lump you mentioned near the anus could be a few things. It might be a hemorrhoid that has not fully resolved, a skin tag, or possibly a thrombosed hemorrhoid. Thrombosed hemorrhoids can occur when blood pools in a hemorrhoid, leading to swelling and pain. If this lump is painful or does not resolve, it is advisable to consult your surgeon or healthcare provider for an evaluation.
To facilitate a more comfortable bowel movement, consider the following tips:
1. Dietary Changes: Increase your fiber intake through fruits, vegetables, and whole grains. This can help soften your stool and make bowel movements easier.
2. Hydration: Drink plenty of water to help keep your stool soft.
3. Stool Softeners: Over-the-counter stool softeners can be beneficial. Consult your doctor before starting any new medication.
4. Sitz Baths: Continue using warm sitz baths several times a day, especially after bowel movements, to soothe the area.
5. Avoid Straining: Try not to strain during bowel movements. If you feel the urge to go, don’t delay it, as waiting can lead to harder stools.
6. Pain Management: Use prescribed pain medications or over-the-counter pain relievers as directed by your healthcare provider to manage discomfort.
If your symptoms persist or worsen, or if you experience severe pain, significant bleeding, or signs of infection (such as fever or increased swelling), it is crucial to seek medical attention promptly. While it may be inconvenient to travel to your surgical center, your health and recovery are paramount.
In summary, while some pain and bleeding are expected after hemorrhoid surgery, it is essential to monitor your symptoms closely. Implementing dietary changes, staying hydrated, and managing pain can aid in your recovery. If you have any concerns or if your symptoms do not improve, do not hesitate to reach out to your healthcare provider for further guidance.
