Combining Oral Baraclude and Long-Acting Interferon for Hepatitis B: Is It Effective? - Gastroenterology and Hepatology

Oral Baraclude combined with injectable long-acting interferon, is this approach feasible?

Hello Director Chen! I have been receiving treatment for hepatitis B at Chang Gung Memorial Hospital since 2008, which has been about 8 years.
I have undergone two courses of Baraclude (a total of six years).
During this time, I reached the criteria for stopping medication twice, but relapsed within six months after discontinuation.
I later used self-paid Baraclude combined with long-acting interferon for six months, and my viral load has remained between 10 and 25.

Question 1: Is it possible that I have developed resistance to Baraclude? Can I currently use Viread combined with interferon?
Question 2: Are there any successful cases of this treatment approach for hepatitis B in Taiwan? I have gradually lost confidence in long-term treatment for hepatitis B.
I am feeling anxious and earnestly request your guidance and valuable advice, so I can regain confidence in continuing to fight the virus.
The revolution has not yet succeeded, and even our founding father could not resist the fate of liver disease.
Should I continue to persevere?

Hello, sir.

1.
It is not drug resistance but rather a common occurrence of relapse after discontinuation of medication.
The significance of using Hepatitis B antiviral therapy remains the same.

2.
In some patients, a combination of oral antiviral therapy and interferon can further reduce the intracellular viral load and decrease the relapse rate after stopping treatment.

3.
For patients who are prone to relapse after discontinuation, it is generally recommended to continue treatment.
If economic considerations necessitate stopping the medication, please ensure regular follow-up to avoid self-harm.

Combining antiviral medications for the treatment of Hepatitis B, such as Baraclude (entecavir) and long-acting interferon, is a topic of considerable interest among healthcare professionals and patients alike. Your experience with Hepatitis B and the challenges you've faced with treatment are not uncommon, and it's understandable to seek clarity on the effectiveness of combination therapies.

Effectiveness of Combination Therapy
1. Antiviral Resistance: Given that you have been on Baraclude for an extended period and have experienced viral rebound after stopping treatment, it is possible that you may have developed some degree of resistance to entecavir. Resistance to antiviral medications can occur, particularly with long-term use, and this can lead to suboptimal viral suppression. The presence of viral load between 10 to 25 IU/mL suggests that the virus is still active, albeit at a low level.
2. Switching to Other Antivirals: Switching to another antiviral, such as Tenofovir (惠立妥), could be a viable option. Tenofovir has a high barrier to resistance and is often recommended for patients who have developed resistance to other nucleos(t)ide analogs. Combining Tenofovir with interferon could potentially enhance the antiviral effect, but this should be done under close medical supervision.

3. Long-Acting Interferon: The use of long-acting interferon (such as Peginterferon) in combination with oral antivirals has shown promise in some studies. Interferon can help boost the immune response against the virus, which may lead to better outcomes, especially in patients who have not achieved sustained viral suppression with nucleos(t)ide analogs alone. However, the side effects associated with interferon therapy, including flu-like symptoms, fatigue, and mood changes, can be significant and should be discussed thoroughly with your healthcare provider.

Clinical Evidence and Success Rates
In Taiwan, as in many parts of the world, there have been successful cases of combination therapy for Hepatitis B. Studies have indicated that combining antiviral therapy with interferon can lead to higher rates of e-antigen seroconversion and sustained virological response compared to using either treatment alone. However, the success of such treatments can vary based on individual patient factors, including the specific viral strain, the presence of liver disease, and overall health.

Maintaining Hope and Confidence
It's understandable to feel disheartened after multiple treatment attempts and relapses. However, it is crucial to maintain open communication with your healthcare provider. They can help you navigate your treatment options, monitor your liver function, and adjust your therapy as needed.
1. Regular Monitoring: Regular follow-ups and blood tests are essential to monitor your liver function and viral load. This will help your doctor make informed decisions about your treatment plan.

2. Support Systems: Engaging with support groups or counseling can also be beneficial. Sharing experiences with others who are facing similar challenges can provide emotional support and practical advice.

3. Staying Informed: Keeping yourself informed about the latest research and treatment options for Hepatitis B can empower you in your treatment journey.

Conclusion
In summary, combining Baraclude with long-acting interferon may be an effective strategy, especially if there is concern about antiviral resistance. Transitioning to Tenofovir could also be a beneficial step. The key is to work closely with your healthcare provider to tailor a treatment plan that is appropriate for your specific situation. Remember, the fight against Hepatitis B is ongoing, and with the right support and treatment, there is hope for a better outcome.
