Cystitis with hematuria after urination?

Hello Dr.
Chen, I would like to ask about a recent situation where I experience bleeding after urination when I exert myself, but I do not have symptoms of frequent urination.
Generally, there is no bleeding after urination, and there is no blood mixed in the urine.
I would like to know if this could be a sign of cystitis.
Also, I would like to know if a medical examination would reveal any signs of sexual activity.

Jin Yu, 20~29 year old female. Ask Date: 2016/07/11

Dr. Chen Jiansheng reply Urology

Post-urination bleeding can be concerning, and while it may suggest a condition like cystitis, it is essential to consider various factors before arriving at a diagnosis. Cystitis, or inflammation of the bladder, often presents with symptoms such as frequent urination, urgency, and pain during urination. However, in your case, the absence of frequent urination and blood in the urine makes cystitis less likely.

Bleeding after urination, especially if it occurs only when straining, could be due to several reasons. One possibility is that there may be irritation or injury to the urethra, which can happen due to various factors, including vigorous sexual activity, trauma, or even certain medical conditions. Another potential cause could be a urinary tract infection (UTI), which might not always present with the classic symptoms of urgency or frequency.
It is also worth noting that conditions such as urethritis (inflammation of the urethra) or even benign conditions like hemorrhoids can cause bleeding that may be mistaken for urinary bleeding. If the bleeding is persistent or accompanied by other symptoms, it is crucial to seek medical attention for a thorough evaluation.

Regarding your concern about whether a medical examination would reveal signs of sexual activity, it is important to understand that healthcare professionals are trained to maintain confidentiality and provide non-judgmental care. If you are concerned about privacy, you can express this to your healthcare provider. They will focus on your symptoms and medical history to provide appropriate care without unnecessary scrutiny of your personal life.

In summary, while your symptoms may suggest a urinary issue, they do not definitively indicate cystitis. It is advisable to consult a healthcare professional for a proper evaluation, which may include a physical examination, urinalysis, and possibly imaging studies to rule out any underlying conditions. Early diagnosis and treatment are essential to prevent complications and ensure your health and well-being.
