Lymphadenopathy since childhood?

Hello doctor, I have a lymph node under my chin that is approximately 2 centimeters in size, and there is also a lymph node on the outer side of my neck that is about 2 centimeters large as well.
These lymph nodes have not decreased in size since I was young.
Now that I am 40 years old, I am concerned that over time there may be changes.
What could be the reason for these lymph nodes not shrinking from childhood to now? Should I go to a major hospital for an examination?

Hong Dou, 40~49 year old female. Ask Date: 2016/07/13

When it comes to persistent lymph node enlargement, especially in the context of your situation where you have had lymph nodes that have remained enlarged for many years, it is important to approach this with a combination of caution and informed decision-making.
Lymph nodes are an integral part of the immune system, and they can become enlarged for various reasons, including infections, inflammation, or malignancies. In your case, the lymph nodes under your chin and on the side of your neck have been consistently enlarged for a long time, which raises a few important considerations.

1. Understanding Persistent Enlargement: Lymph nodes can remain enlarged due to chronic conditions such as recurrent infections or autoimmune diseases. In some cases, they may also be a sign of a more serious underlying condition, including lymphoma or other malignancies. However, many individuals have benign conditions that cause lymph node enlargement without any serious implications.
2. When to Seek Medical Advice: Given that your lymph nodes have been enlarged for many years without any significant changes, it is advisable to consult with a healthcare professional, particularly if you notice any new symptoms such as weight loss, night sweats, fever, or persistent fatigue. These symptoms could indicate a need for further evaluation.
3. Diagnostic Evaluation: It would be prudent to visit a healthcare provider, preferably an ear, nose, and throat (ENT) specialist or a general practitioner who can assess your condition. They may recommend imaging studies such as an ultrasound or CT scan to evaluate the characteristics of the lymph nodes. If there are any concerning features, a biopsy may be warranted to rule out malignancy.

4. Follow-Up and Monitoring: If your healthcare provider determines that the lymph nodes are stable and there are no alarming features, they may suggest a watchful waiting approach with regular follow-ups. This means monitoring the lymph nodes over time to ensure they do not change in size or character.

5. Importance of Context: The context of your overall health is crucial. If you have a history of recurrent infections or other health issues, this may provide insight into why your lymph nodes remain enlarged. Additionally, family history and any other symptoms you may have experienced should be discussed with your healthcare provider.

6. Surgical Considerations: If the lymph nodes are found to be suspicious or if they continue to grow, surgical intervention may be necessary to obtain a definitive diagnosis. This could involve a biopsy or, in some cases, removal of the lymph node.

In summary, while persistent lymph node enlargement can be benign, it is essential to seek medical advice to rule out any serious conditions. A thorough evaluation by a healthcare professional will provide clarity and guide you on the appropriate next steps. It is always better to err on the side of caution, especially when it comes to changes in your health that have persisted over time.
