Foreskin issues

Hello, Dr.
Hsu.
I previously underwent two treatments for balanitis.
Recently, I noticed a lump on the outer side of the penile foreskin that resembles the image at this link: http://www.eskin.com.tw/DB/Upload_SickOnline/L20110925224511.jpg.
It itches when I touch it.
Could this be scabies or another skin condition? Additionally, the glans and foreskin are not itchy, but there is still swelling around the foreskin.
What can I do to improve this condition? Thank you.

A Da, 20~29 year old female. Ask Date: 2016/07/14

When it comes to penile skin issues, particularly conditions like balanitis (inflammation of the glans) or other skin irritations, it's essential to approach the situation with care and proper medical guidance. Based on your description, it seems you are experiencing some discomfort and possibly a rash or irritation on the outer side of the foreskin. This could be due to a variety of reasons, including infections, allergic reactions, or skin conditions.

Symptoms and Possible Conditions
1. Balanitis: This is an inflammation of the glans (the head of the penis) and can be caused by infections (bacterial or fungal), poor hygiene, or irritants. Symptoms often include redness, swelling, itching, and sometimes discharge.

2. Contact Dermatitis: This can occur due to irritation from soaps, lotions, or other products that come into contact with the skin. Symptoms typically include redness, itching, and sometimes blistering.

3. Fungal Infections: Conditions like candidiasis (yeast infection) can also cause similar symptoms, particularly if there is an overgrowth of yeast in the area.

4. Psoriasis or Eczema: These chronic skin conditions can also affect the genital area, leading to red, itchy patches.

5. Scabies: While less common, scabies can cause intense itching and a rash due to a mite infestation. However, this typically presents with more widespread symptoms rather than localized irritation.

Treatment Options
1. Maintain Hygiene: Ensure that the area is kept clean and dry. Gently wash with mild soap and water, and avoid harsh soaps or products that may irritate the skin.

2. Topical Treatments: Depending on the diagnosis, a healthcare provider may recommend topical antifungal creams for fungal infections, hydrocortisone cream for inflammation, or other medicated creams for specific conditions.

3. Avoid Irritants: Identify and avoid any products that may be causing irritation, such as scented soaps, lotions, or condoms that contain allergens.

4. Consult a Dermatologist or Urologist: Given the symptoms you are experiencing, it is crucial to consult a healthcare professional for an accurate diagnosis. They may perform a physical examination and possibly take a sample for further testing to determine the exact cause of the irritation.

5. Oral Medications: If an infection is diagnosed, oral antifungal or antibiotic medications may be prescribed.

6. Lifestyle Adjustments: Wearing loose-fitting, breathable underwear can help reduce moisture and irritation. Additionally, maintaining a healthy lifestyle, including a balanced diet and proper hydration, can support skin health.

Conclusion
It’s important to avoid self-diagnosing or self-treating, as the genital area is sensitive and can be affected by various conditions. A healthcare professional can provide a thorough examination and appropriate treatment options tailored to your specific situation. If you notice any worsening of symptoms, such as increased redness, swelling, or discharge, seek medical attention promptly. Remember, early intervention can often lead to better outcomes and relief from discomfort.
