Do I need a follow-up appointment?

Hello, doctor! At the gynecology clinic, a skin biopsy and the dermatology clinic's examination both indicated that it was warts.
This past Monday, I was prescribed medication for the warts, but the itching and stinging sensations persisted from before the consultation until after starting the wart medication.
I then visited the Dalin Tzu Chi gynecology clinic, where the doctor stated that it was not warts but rather a recurrent infection caused by a sexually transmitted bacterium that flares up due to staying up late or heat.
After applying the medication prescribed by the Tzu Chi doctor, the itching and stinging significantly improved.

[Medication] Generic name: U66 Nystatin/Neomycin/Triamcinolone Brand name: Mycomb cream Indication: Treatment of infections, antifungal, and anti-inflammatory.
Given that my symptoms have improved, should I still see another doctor or undergo testing for warts? Before going to the hospital, I wanted to ask for your opinion.
Thank you for your response! :)

Dr. Huang Yiwen reply Obstetrics and Gynecology

Hello, Miss.
Although your itching and tingling symptoms have improved, it is still important to check whether the lesions on your skin have disappeared.
The diagnosis should be based on the pathology report from a skin biopsy.
It is advisable to confirm the pathology report and consider seeking a second opinion from another physician.

More Info

Based on your description, it seems that you have been experiencing persistent skin issues that have led you to seek medical advice from multiple healthcare providers. The initial diagnosis of "cauliflower" (likely referring to genital warts caused by the human papillomavirus, HPV) was later contradicted by another physician who suggested that your symptoms were due to a different, non-viral skin condition exacerbated by factors such as lack of sleep and heat. You have also been prescribed a topical cream (Mycomb cream) that contains Nystatin, Neomycin, and Triamcinolone, which is used for treating infections, fungal conditions, and inflammation.

To address your question about whether you need a follow-up appointment, it is essential to consider a few factors:
1. Symptom Improvement: You mentioned that after using the prescribed cream, you noticed a significant improvement in your itching and pain. This is a positive sign, indicating that the treatment may be effective for your condition. However, it is crucial to monitor your symptoms closely. If they return or worsen, a follow-up appointment would be advisable.

2. Diagnosis Clarity: Since there was conflicting information regarding your diagnosis, it may be beneficial to seek a follow-up appointment with a dermatologist or a specialist who can provide a definitive diagnosis. This is particularly important if you are still uncertain whether your condition is viral or due to another cause. A clear diagnosis will guide appropriate treatment and management strategies.

3. Potential for Recurrence: Skin conditions can often recur, especially if the underlying causes (such as irritation, heat, or other environmental factors) are not addressed. If you find that your symptoms return after stopping the use of the cream, it may be necessary to revisit your healthcare provider for further evaluation and possibly a different treatment plan.

4. Preventive Measures: In addition to treating the current symptoms, it is also essential to discuss preventive measures with your healthcare provider. This may include lifestyle changes, skincare routines, or other interventions to minimize the risk of recurrence.

5. Follow-Up Testing: If there is still concern about the possibility of HPV or other sexually transmitted infections (STIs), it may be prudent to undergo further testing. This is especially true if you have had multiple partners or if there is a history of STIs in your sexual history.

In conclusion, while your symptoms have improved with the current treatment, it is advisable to schedule a follow-up appointment to ensure that the underlying cause of your skin condition is accurately diagnosed and managed. This will help prevent future flare-ups and ensure that you receive the most effective treatment tailored to your specific needs. Always prioritize open communication with your healthcare provider regarding any changes in your symptoms or concerns about your skin condition.
