Wisdom Tooth Surgery Post-Operative Care

Hello, doctor.
My wisdom tooth is impacted due to its improper alignment and hasn't erupted.
During my dental cleaning, the doctor recommended surgery for extraction based on the X-ray.
Last Friday, I had the wisdom tooth removed, and now, on the fourth day post-surgery, the stitches haven't come out, but the wound appears to be opening up.
However, I don't feel any pain.
Should I seek medical attention immediately? I have also used bone graft material and collagen membrane.

Miss Zhang, 10~19 year old female. Ask Date: 2016/07/16

Dr. Lin Jiayong reply Dentistry

Hello: Since you have used bone powder and collagen, which are both relatively expensive materials, it is recommended that you return to the original clinic for continuous follow-up, regardless of whether the wound has opened or not.
Thank you! Sincerely, Lin Chia-Yung, Director of Dentistry, Taichung Hospital, Ministry of Health and Welfare.

After wisdom tooth extraction, it is crucial to monitor your recovery closely, as complications can arise even if you are not experiencing pain. In your case, you mentioned that the extraction site appears to be opening up, which could indicate a potential issue such as a dry socket or infection, even in the absence of pain. Here are some key points to consider regarding when to seek medical attention after wisdom tooth extraction:
1. Signs of Infection: While pain is a common symptom of infection, it is not the only indicator. Other signs include swelling, redness, warmth around the extraction site, and the presence of pus or unusual discharge. If you notice any of these symptoms, it is advisable to contact your dentist or oral surgeon.

2. Dry Socket: This condition occurs when the blood clot at the extraction site either dissolves or becomes dislodged, exposing the underlying bone and nerves. Symptoms of dry socket may include a foul taste in the mouth, bad breath, and pain that may radiate to the ear or jaw. If you suspect you have a dry socket, you should seek medical attention promptly, as it can delay healing and cause significant discomfort.

3. Wound Healing: The fact that your stitches have not yet dissolved and the wound appears to be opening could be a concern. While some degree of healing is expected, any significant opening or separation of the wound may require evaluation by your dentist. They can assess whether the stitches need to be adjusted or if additional treatment is necessary.

4. Bone Grafting Material: Since you mentioned that you had bone grafting material (bone powder and collagen) placed at the extraction site, it is essential to ensure that this material is integrating properly and that there are no complications related to it. If you notice any unusual symptoms, such as increased swelling or discomfort, it is best to consult your dentist.

5. Follow-Up Appointments: Most oral surgeons or dentists will schedule a follow-up appointment after wisdom tooth extraction to monitor healing. If you have not had a follow-up yet, or if your next appointment is not for a while, consider reaching out to your dental office for guidance on whether you should come in sooner.

In summary, while the absence of pain is a positive sign, the appearance of the extraction site warrants attention. It is always better to err on the side of caution when it comes to your health. If you are uncertain or anxious about your recovery, do not hesitate to contact your dentist or oral surgeon for advice. They can provide you with the best guidance based on your specific situation and ensure that any potential issues are addressed promptly. Remember, timely intervention can prevent complications and facilitate a smoother recovery process.
