Persistent Facial Numbness and Swelling Eight Months After Car Accident - Surgery

Eight months after the car accident, there is still facial numbness and swelling?

Hello, Doctor! It has been eight months since my car accident.
The diagnosis from the accident was multiple fractures around the zygomatic bone, with bone cracks compressing the nerves.
I believe the fractures have healed by now.
However, the area from the right side of my nose to my mouth still feels numb and swollen to the touch, and there is a noticeable asymmetry when I look in the mirror.
My dental occlusion still feels off as well.
I have been consistently taking B12 supplements and applying warm compresses, but it seems there hasn't been much progress over the past few months.
Doctor, can the facial nerves recover? Will the swelling in my face completely subside? I'm really worried that my face will remain asymmetrical like this...
I'm feeling quite discouraged due to the lack of significant progress in the past few months.
Thank you!

Hello! A follow-up CT scan should be performed to determine if healing has occurred.
There is no need for heat compresses, as they may increase swelling.
Nerve recovery will take some time, and it may not return to completely normal.
The facial swelling will also take time to subside, and sometimes it may still appear swollen even after a year.
Take care! Changhua Hospital is concerned about you!

After a car accident, it is not uncommon for individuals to experience persistent symptoms such as facial numbness, swelling, and asymmetry, especially when there has been trauma to the facial bones and surrounding nerves. In your case, the diagnosis of multiple fractures around the zygomatic area (cheekbone) and the associated nerve compression can lead to long-lasting effects on facial sensation and appearance.

The facial nerve, which controls the muscles of facial expression and provides sensation to parts of the face, can be affected by trauma. When the nerve is compressed or injured, it may lead to symptoms such as numbness, tingling, or weakness in the affected areas. The fact that you are experiencing persistent numbness and swelling eight months post-accident suggests that the nerve may still be recovering or that there may be ongoing inflammation or scarring in the area.

Regarding your concerns about the recovery of facial nerves and the potential for swelling to subside, it is important to understand that nerve healing can be a slow process. In some cases, nerves can take months or even years to fully recover, and the extent of recovery can vary significantly from person to person. Factors that can influence recovery include the severity of the injury, the presence of any ongoing inflammation, and individual healing capacities.

You mentioned that you have been taking vitamin B12 and applying heat, which are both supportive measures. Vitamin B12 is known to play a role in nerve health, and heat can help improve blood flow to the area, potentially aiding in healing. However, if you have not seen significant improvement, it may be beneficial to consult with a specialist, such as a neurologist or a facial plastic surgeon, who can assess your condition more thoroughly.

In terms of facial asymmetry, it is possible that some degree of asymmetry may persist, especially if there was significant structural damage to the bones or nerves. However, many individuals experience improvement over time as swelling decreases and nerve function gradually returns. If the asymmetry is pronounced and concerning, surgical options may be available to correct it, but this would depend on a thorough evaluation by a qualified professional.

It is also worth considering physical therapy or rehabilitation focused on facial exercises, which can help improve muscle tone and coordination in the affected areas. This can be particularly beneficial if there is any weakness in the facial muscles due to nerve involvement.

In summary, while it is understandable to feel discouraged by the lack of progress, it is important to remain hopeful. Nerve recovery can take time, and there are various avenues for treatment and support. Consulting with specialists who can provide a comprehensive evaluation and tailored treatment plan will be crucial in addressing your ongoing symptoms and concerns about facial appearance. Remember to keep an open line of communication with your healthcare providers, as they can offer guidance and support throughout your recovery journey.
