Testicular issues
I currently have a condition where my left testicle is larger than my right one, and the left side is significantly bigger.
This phenomenon has been present for a long time, but I haven't experienced any discomfort.
When I palpate, I can feel something soft on top of the left testicle that feels somewhat like a collection of small strands, but they are all together, and it feels quite substantial.
The right testicle also has something similar, but it feels much smaller.
I previously asked about this on a knowledge-sharing platform, but I've started to worry again.
The size difference between my testicles is quite noticeable, unlike what I read online about them being relatively similar.
I would like to know if this is considered a normal phenomenon, and what that soft structure on top of the testicle might be.
This condition has been present for about 1 to 2 years.
When I palpate, there is no pain or sharp sensation, although I occasionally experience acute pain in the testicles, but that hasn't happened in a long time.
Thank you, doctor, for your response.

Dr. Sun Hongshun reply Urology
Hello: It is normal for the left and right testicles to be of different sizes.
The structure above the testicle is the epididymis.
Every male is advised not to excessively stimulate the testicles.
It is recommended to wear loose-fitting pants.
If you still have questions, please consult a urologist.

When it comes to testicular size differences, it's important to understand that some degree of asymmetry is quite common among men. In fact, many men have one testicle that is slightly larger or hangs lower than the other. However, significant differences in size, especially if they have developed over time, can warrant further investigation.
In your case, you mentioned that your left testicle is noticeably larger than the right and that you have felt a soft, stringy structure on the surface of the left testicle. This could potentially be the epididymis, which is a coiled tube located at the back of each testicle that stores and matures sperm. The epididymis can sometimes feel prominent, especially if there is a variation in testicular size. However, if this structure feels unusually large or if you notice any changes in texture or shape, it is advisable to consult a healthcare professional.
The acute pain you mentioned experiencing occasionally could be related to several factors, including testicular torsion, epididymitis (inflammation of the epididymis), or even a hernia. While you have not experienced pain recently, the history of acute pain should not be overlooked. It is crucial to monitor any changes in your symptoms, such as the onset of pain, swelling, or changes in the size of the testicles.
In general, if you notice a significant change in size, shape, or if you experience any discomfort, it is essential to seek medical advice. A healthcare provider may perform a physical examination and possibly an ultrasound to assess the condition of your testicles and rule out any underlying issues, such as tumors or cysts.
In summary, while some asymmetry in testicular size is normal, significant differences or the presence of unusual structures should be evaluated by a medical professional. Regular self-examinations can help you become familiar with your anatomy and detect any changes early. If you have any concerns about your testicular health, do not hesitate to reach out to a urologist or your primary care physician for further evaluation and peace of mind.
