Chest tightness or pain?
Recently, I have been experiencing frequent chest tightness and a sensation of pressure in my throat, as if I need to burp but can't.
When I try to burp, it feels like there are bubbles that want to come up but don't.
This has been going on for almost a month.
I'm not sure if it's just a stomach issue or something related to the chest cavity.
Which specialist should I see?

Dr. Jian Shuntian reply Pulmonology
Hello: Based on the symptoms, it may be a gastroesophageal reflux issue.
Please first consult a pulmonologist.
If there are no issues, you may seek further advice from the pulmonologist regarding whether to see a gastroenterologist, cardiologist, or another specialist.
Thank you.
Best regards.

Chest pain can be a concerning symptom, and it’s important to understand the potential causes and when to seek medical attention. Based on your description of persistent chest discomfort and the sensation of needing to clear your throat, it’s crucial to consider both gastrointestinal and cardiac issues, as both can present with similar symptoms.
Firstly, the sensation of chest tightness or discomfort, along with a feeling of needing to clear your throat, can often be related to gastrointestinal problems, such as gastroesophageal reflux disease (GERD). GERD occurs when stomach acid flows back into the esophagus, leading to symptoms like heartburn, chest pain, and a sensation of a lump in the throat. This condition can cause significant discomfort and may mimic cardiac pain, making it essential to differentiate between the two.
On the other hand, chest pain can also be indicative of cardiac issues, such as angina or even a heart attack. Angina is characterized by chest pain or discomfort that occurs when the heart muscle doesn’t get enough oxygen-rich blood. This pain can feel like pressure, squeezing, or fullness in the chest and may radiate to the arms, neck, jaw, or back. Given that you have experienced this discomfort for nearly a month, it is vital to rule out any serious cardiac conditions.
In your case, since the symptoms have persisted for an extended period, it is advisable to consult a healthcare professional. You should consider seeing a primary care physician first, who can perform an initial evaluation. They may conduct a physical examination, take a detailed medical history, and possibly order tests such as an electrocardiogram (ECG), chest X-ray, or blood tests to assess your heart function and rule out any acute conditions.
If your primary care physician suspects a gastrointestinal issue, they may refer you to a gastroenterologist for further evaluation. This specialist can perform tests such as an upper endoscopy or esophageal pH monitoring to diagnose conditions like GERD or esophagitis.
If your symptoms are more consistent with cardiac issues, your physician may refer you to a cardiologist. This specialist can conduct more specific tests, such as a stress test or echocardiogram, to evaluate the health of your heart and blood vessels.
In summary, given the duration and nature of your symptoms, it is essential to seek medical attention promptly. Start with a visit to your primary care physician, who can guide you through the appropriate steps and referrals based on their findings. Remember, it’s always better to err on the side of caution when it comes to chest pain, as timely intervention can be critical in addressing any underlying health issues.
