Post-operative bleeding issues one week after hemorrhoidectomy?
Hello, Doctor.
I had a hemorrhoidectomy at Tri-Service General Hospital a week ago.
I started having bowel movements on the third day post-surgery, but on the fourth night, after passing gas, I noticed blood staining my outer pants (the stain was about five centimeters in diameter) and there were also some blood clots on my underwear.
On the fifth day, I could clearly see blood mixed in with my stool.
The sixth day was uneventful, but on the seventh night (shortly before posting this), I experienced a situation similar to the fourth day, and it seemed like there was a bit more blood.
There are still several days left for my recovery.
I have been taking my medication on schedule.
I would like to ask Dr.
Ke if this situation is normal.
Thank you!

After hemorrhoid excision surgery, it is possible to experience bleeding.
Generally, medication can alleviate the bleeding.
However, if the bleeding does not stop, a return to the operating room for suturing and hemostasis may be necessary.
It is recommended that you return to the outpatient clinic for a physician evaluation to prevent severe bleeding that could pose a risk to life.

Post-surgery bleeding after a hemorrhoidectomy can be a concerning issue for many patients, and it’s important to understand what is considered normal and when to seek further medical attention. Based on your description, it seems that you have experienced some bleeding and blood clots after your surgery, which is not uncommon. However, the amount and frequency of bleeding you are experiencing warrant careful consideration.
After a hemorrhoidectomy, it is typical for patients to experience some bleeding, especially during the first week post-surgery. This bleeding can occur due to several factors, including the healing process of the surgical site, the presence of stool passing through the rectum, and the potential for irritation or trauma to the surgical area. Light spotting or small amounts of blood mixed with stool can be expected, particularly in the initial days following the procedure.
However, the fact that you have noticed a significant amount of blood—such as a five-centimeter diameter stain on your underwear and blood clots in your stool—raises some concerns. While some bleeding is normal, large amounts of blood or the presence of clots can indicate complications such as:
1. Infection: Surgical sites can become infected, leading to increased bleeding and discharge.
2. Hemorrhage: If the bleeding is more than just light spotting, it could indicate a hemorrhage, which may require medical intervention.
3. Delayed Healing: If the surgical site is not healing properly, it may lead to increased bleeding.
Given that you have experienced bleeding on multiple occasions, it is crucial to consult your surgeon or healthcare provider as soon as possible. They may want to examine the surgical site to ensure that there are no complications such as an infection or a hematoma (a localized collection of blood outside of blood vessels).
In terms of your medication, it is essential to follow your doctor's instructions regarding pain management and any prescribed medications. Some medications, particularly blood thinners or anti-inflammatory drugs, can increase the risk of bleeding. If you have concerns about how your medications may be affecting your bleeding, discuss this with your healthcare provider.
Additionally, you mentioned that you have been practicing good post-operative care, such as taking sitz baths and applying ointments. These practices can help soothe the area and promote healing, but they should not replace medical evaluation if you are experiencing significant bleeding.
In summary, while some bleeding after a hemorrhoidectomy can be normal, the amount and frequency you describe suggest that you should seek medical advice promptly. Your healthcare provider can assess your situation and determine if any further treatment is necessary. Remember, it is always better to err on the side of caution when it comes to post-surgical complications.
