Hair thinning?

Hello Dr.
Liao: I have had thin hair since childhood, and my hair texture is very fine.
Because of this, I permed my hair at the beginning of July this year, but it didn't last long, so I permed it again at the beginning of October (a total of two times).
However, it seems that my hair has been getting thinner, and sometimes I notice hair falling out when I gently comb it.
I'm not sure if this is largely related to my daily hair washing.
Could you please advise me on how to improve this situation?

Chen, 10~19 year old female. Ask Date: 2000/10/31

Dr. Liao Wenyu reply Dermatology

Hello: The amount and quality of hair are significantly related to one's constitution.
It is normal to lose 100-200 hairs a day; some additional shedding occurs during brushing and washing.
As long as the amount lost does not exceed this range, it is considered normal.

Improving thinning hair can be a multifaceted approach that requires understanding the underlying causes, adopting proper hair care practices, and possibly utilizing medical treatments. Here are some tips and recommendations based on dermatological insights:

Understanding Thinning Hair
Thinning hair can be attributed to various factors, including genetics, hormonal changes, nutritional deficiencies, stress, and hair care practices. If you have a family history of hair loss, you may be experiencing androgenetic alopecia, commonly known as male or female pattern baldness. This condition is influenced by genetic predisposition and hormonal levels, particularly androgens.

Hair Care Practices
1. Gentle Washing: Washing your hair daily can strip it of natural oils, leading to dryness and brittleness. Instead, consider washing your hair every other day or even less frequently, depending on your scalp's oiliness. Use a mild, sulfate-free shampoo that is gentle on the hair and scalp.

2. Avoid Harsh Treatments: Frequent chemical treatments, such as perming or coloring, can weaken hair strands and lead to breakage. If you must style your hair, opt for less damaging methods and allow your hair time to recover between treatments.

3. Proper Drying Techniques: After washing, avoid vigorous towel-drying, which can cause breakage. Instead, gently pat your hair dry or use a microfiber towel. When using heat styling tools, apply a heat protectant spray to minimize damage.

4. Scalp Care: A healthy scalp is crucial for hair growth. Avoid scratching your scalp, as this can lead to irritation and inflammation. If you notice oil buildup or dandruff, consider using a gentle exfoliating scalp scrub or a medicated shampoo designed to treat these conditions.

Nutritional Considerations
A balanced diet plays a significant role in hair health. Ensure you are consuming adequate amounts of:
- Proteins: Hair is primarily made of protein (keratin), so include lean meats, fish, eggs, legumes, and dairy in your diet.

- Vitamins and Minerals: Vitamins A, C, D, E, and B vitamins (especially biotin) are essential for hair growth. Minerals like iron and zinc are also crucial. Consider incorporating fruits, vegetables, nuts, and whole grains into your meals.

Medical Treatments
If you are experiencing significant hair thinning, it may be beneficial to consult a dermatologist. They can assess your condition and recommend appropriate treatments, which may include:
1. Minoxidil (Rogaine): This over-the-counter topical solution is FDA-approved for hair regrowth in both men and women. It works by increasing blood flow to hair follicles, promoting hair growth.

2. Finasteride (Propecia): This prescription medication is primarily for men and works by inhibiting the hormone responsible for hair loss. It is not recommended for women, especially those who are pregnant or may become pregnant.

3. Platelet-Rich Plasma (PRP) Therapy: This is a newer treatment where your blood is drawn, processed, and injected into the scalp to stimulate hair growth.

4. Hair Transplant Surgery: For those with significant hair loss, surgical options like hair transplants may be considered. This involves moving hair follicles from a donor site to the thinning areas.

Stress Management
Stress can exacerbate hair loss, so incorporating stress-reducing practices such as yoga, meditation, or regular exercise can be beneficial. Additionally, ensuring you get enough sleep is vital for overall health and can positively impact hair growth.

Conclusion
Improving thinning hair requires a holistic approach that combines proper hair care, nutrition, medical treatments, and stress management. If you are concerned about your hair loss, it is advisable to consult a dermatologist who can provide personalized recommendations based on your specific situation. Remember, patience is key, as hair growth can take time, and results may not be immediately visible.
