Abnormal secretion color?
Hello Doctor: I have been experiencing yellow-green discharge for over two years.
The symptoms start about a week after my menstrual period ends, and I have become a regular visitor to the gynecologist (since I go every month).
Sometimes there is itching, and the discharge can be thick or thin, resembling small curds.
I frequently have yeast infections, and I am not on birth control and do not have diabetes! Yet, I continue to have recurrent infections.
Each time, the treatment consists of suppositories and oral antibiotics (I have been diligent about using the suppositories and taking the medication), and I also do sitz baths, but I still experience infections every month.
I have discussed with my doctor the need for a discharge test, but the response has always been, "Why do you need a test? You don't need to be tested! This is just a common fungal infection!" They insist that I do not need testing.
Doctor, what should I do? I am feeling so hopeless with these recurring episodes every month!

Dr. Lin Manying reply Obstetrics and Gynecology
If recurrent episodes occur over a long period, it may cause itching of vaginal discharge, and it is advisable to undergo a Pap smear.
If there is sexual activity, it is recommended that the boyfriend may also need treatment to restore the vaginal environment to its normal weakly acidic state.
Reduce late-night activities, limit sugar intake, and consume probiotics; consistency is key.

Persistent yellow-green discharge can be a frustrating and distressing issue, especially when it becomes a recurring problem over an extended period. Based on your description, it seems that you have been experiencing these symptoms for over two years, with a pattern of discharge that varies in consistency and color, sometimes accompanied by itching and discomfort. This situation can be indicative of chronic vaginal infections, which may involve various pathogens, including yeast, bacteria, or sexually transmitted infections (STIs).
Firstly, it is essential to understand that vaginal discharge can vary significantly in color, consistency, and odor based on several factors, including hormonal changes, menstrual cycle phases, and the presence of infections. Yellow-green discharge, particularly if it has a foul odor, can often suggest a bacterial infection, such as bacterial vaginosis or a sexually transmitted infection like trichomoniasis. The presence of clumpy or cottage cheese-like discharge may indicate a yeast infection, commonly caused by Candida species.
Given that you have been experiencing these symptoms consistently for two years and have undergone multiple treatments without lasting relief, it is crucial to consider a few key points:
1. Comprehensive Evaluation: It sounds like you have been diligent in seeking medical advice, but it may be beneficial to seek a second opinion from a different healthcare provider, preferably one who specializes in gynecology or infectious diseases. A thorough evaluation, including a detailed history and physical examination, is essential to identify the underlying cause of your symptoms.
2. Laboratory Testing: While your current provider may believe that your symptoms are due to a common yeast infection, it is essential to conduct specific laboratory tests to rule out other potential causes. This may include a vaginal swab for culture and sensitivity testing, which can help identify the specific organism responsible for your symptoms. Additionally, testing for STIs, including chlamydia and gonorrhea, should be considered, especially if there is any history of unprotected sexual contact.
3. Consider Underlying Conditions: Chronic infections can sometimes be linked to underlying conditions such as hormonal imbalances, immune system issues, or even anatomical abnormalities. Discussing these possibilities with your healthcare provider may help guide further investigations.
4. Treatment Options: If you are diagnosed with a specific infection, appropriate treatment should be initiated. This may involve antifungal medications for yeast infections, antibiotics for bacterial infections, or other targeted therapies based on the identified pathogen. It is also crucial to follow the prescribed treatment regimen thoroughly and to complete the entire course of medication, even if symptoms improve before finishing the treatment.
5. Lifestyle and Hygiene Practices: Maintaining proper vaginal hygiene can also play a role in managing symptoms. Avoiding irritants such as scented soaps, douches, and tight-fitting clothing can help reduce irritation. Additionally, wearing breathable cotton underwear and practicing good hygiene can support vaginal health.
6. Follow-Up Care: Regular follow-up appointments with your healthcare provider are essential to monitor your condition and adjust treatment as necessary. If your symptoms persist despite treatment, further evaluation may be warranted to explore other potential causes.
In conclusion, persistent yellow-green discharge accompanied by itching and discomfort warrants a comprehensive evaluation to identify the underlying cause. Seeking a second opinion and undergoing appropriate laboratory testing can provide valuable insights into your condition. With the right approach, it is possible to manage and alleviate your symptoms effectively. Remember, you are not alone in this, and there are healthcare professionals who can help you navigate this challenging situation.
