Pain and Hardness Around the Nipple: Possible Causes - Surgery

Nipple papilloma, pain, and surrounding hardness?

Hello doctor, yesterday I suddenly experienced pain in my right nipple.
Compared to the left side, the right side has also darkened in color.
Normally, I don't feel much discomfort, but there is pain when there is friction or pressure.
Today, I noticed that the area around the right nipple and areola is red and swollen, and there is a hard lump that can be felt when pressed, which is also painful.
Doctor, what could be the possible causes of this?

Dr. Chen Jiaming reply Surgery

In Taiwan, approximately 10,000 new cases of breast cancer are diagnosed each year, with only about 30-40 cases occurring in men.
Male breast cancer is most commonly found in older adults (ages 60-80).
In younger males, nipple symptoms are often due to inflammation, as the areola has hair and sebaceous glands, which can lead to soft tissue infections.
Typically, oral antibiotics are effective for treatment.

When experiencing sudden pain and changes in color around the nipple, particularly with accompanying symptoms such as swelling, tenderness, and the presence of a hard lump, it is essential to consider several potential causes. The symptoms you describe could be indicative of various conditions, ranging from benign to more serious issues. Here are some possible explanations:
1. Infection or Inflammation: One of the most common causes of localized pain and swelling around the nipple is an infection, such as mastitis or a breast abscess. Mastitis is an infection of the breast tissue that results in breast pain, swelling, warmth, and redness. It is often associated with breastfeeding but can occur in non-lactating women as well. If the area is warm to the touch and you have systemic symptoms like fever, this could be a significant concern.

2. Ductal Ectasia: This condition occurs when a milk duct beneath the nipple dilates, fills with fluid, and thickens. It can lead to inflammation and infection, resulting in pain and changes in the appearance of the nipple. This condition is more common in women who are nearing menopause.

3. Fibrocystic Changes: Many women experience fibrocystic breast changes, which can cause lumps, tenderness, and pain in the breast tissue. These changes are often related to hormonal fluctuations and can be cyclical, worsening before menstruation.

4. Nipple Trauma or Irritation: If there has been any recent trauma to the breast or nipple area, such as friction from clothing or vigorous physical activity, this could lead to localized pain and swelling. Additionally, skin conditions like eczema or contact dermatitis can cause irritation and changes in color.

5. Breast Cysts or Tumors: While less common, the presence of a lump could indicate a cyst or, in rare cases, a benign or malignant tumor. Breast cysts are fluid-filled sacs that can cause discomfort, while tumors may present as hard lumps that are painful or painless.

6. Paget's Disease of the Nipple: This is a rare form of breast cancer that typically presents with changes in the skin and appearance of the nipple, including redness, scaling, and discharge. It is often associated with underlying breast cancer.

Given the symptoms you are experiencing, it is crucial to seek medical attention promptly. A healthcare provider will likely perform a thorough examination, which may include imaging studies such as a mammogram or ultrasound, to assess the underlying cause of your symptoms. They may also consider a biopsy if there is any suspicion of malignancy.

In the meantime, it is advisable to avoid any irritants, such as tight clothing or harsh soaps, and to monitor for any changes in symptoms. If you experience fever, increased redness, or spreading of the swelling, seek immediate medical care, as these could be signs of a more serious infection.

In summary, while there are several potential causes for the pain and hardness around your nipple, a thorough evaluation by a healthcare professional is necessary to determine the exact cause and appropriate treatment. Early diagnosis and intervention can lead to better outcomes, especially if an underlying condition requires specific management.
