Dental issues
Hello, doctor.
I noticed some localized white spots on the surface of a few of my upper teeth while brushing in the past few days.
I've been undergoing orthodontic treatment for over four months, and I'm not sure if this is normal.
Since it's currently summer vacation, I can't visit my original clinic, which makes me a bit worried! Thank you, doctor.

Dr. Lin Jiayong reply Dentistry
Hello: The localized white spots are most likely caused by demineralization of the teeth.
It is recommended that you enhance your oral hygiene practices and use fluoride toothpaste more frequently.
Of course, the most important thing is to schedule a follow-up appointment as soon as possible.
Wishing you safety and good health.

White spots on teeth during orthodontic treatment can be a common concern for many patients. These spots, often referred to as "white spot lesions," are typically a sign of demineralization of the enamel, which can occur due to various factors, especially during orthodontic treatment. Here’s a detailed explanation of what these white spots are, why they occur, and what you can do about them.
Understanding White Spots
White spots on teeth are areas where the enamel has lost minerals, leading to a change in color. This condition is often associated with poor oral hygiene, especially when braces or aligners are involved. The brackets and wires can make it more challenging to clean the teeth effectively, allowing plaque to build up around these areas. If plaque is not removed through proper brushing and flossing, it can lead to the demineralization of the enamel, resulting in white spots.
Causes of White Spots During Orthodontic Treatment
1. Plaque Accumulation: Braces create additional surfaces for plaque to adhere to. If these areas are not cleaned properly, it can lead to the formation of white spots.
2. Diet: Consuming acidic or sugary foods and beverages can contribute to enamel erosion. This is particularly concerning for orthodontic patients who may indulge in snacks that are not conducive to oral health.
3. Oral Hygiene Practices: Inadequate brushing and flossing can lead to plaque buildup. It’s crucial to maintain a rigorous oral hygiene routine, especially when wearing braces.
4. Fluoride Exposure: While fluoride is beneficial for preventing cavities, excessive fluoride exposure can also lead to a condition known as dental fluorosis, which can manifest as white spots on the teeth.
What to Do About White Spots
1. Improve Oral Hygiene: Ensure that you are brushing at least twice a day with fluoride toothpaste and flossing daily. Consider using an interdental brush or a water flosser to clean around braces effectively.
2. Regular Dental Check-ups: Even if you are on break from your orthodontic treatment, it’s essential to visit your dentist for regular check-ups. They can provide professional cleanings and monitor the health of your teeth.
3. Dietary Adjustments: Limit sugary snacks and acidic foods. Instead, focus on a balanced diet rich in fruits, vegetables, and dairy products, which can help strengthen enamel.
4. Fluoride Treatments: Ask your dentist about fluoride treatments or remineralization products that can help restore minerals to the enamel and reduce the appearance of white spots.
5. Consult Your Orthodontist: If you are particularly concerned about the white spots, reach out to your orthodontist. They may be able to provide specific recommendations or treatments to address the issue.
Conclusion
While white spots on teeth during orthodontic treatment can be concerning, they are often manageable with proper care and attention. Maintaining excellent oral hygiene, being mindful of your diet, and seeking professional advice when needed can help prevent further demineralization and keep your teeth healthy throughout your orthodontic journey. If you notice any changes or worsening of the spots, do not hesitate to consult with your dentist or orthodontist for tailored advice and treatment options.
