Postoperative care for varicocele surgery?
Hello Dr.
Fu: Due to family planning, we have experienced infertility during the two years after marriage and have undergone examinations, which revealed that my husband has a varicocele issue.
He underwent varicocele surgery in mid-March of this year, and he has been able to engage in normal physical activities post-surgery.
However, it has been almost six months, and he still experiences a tingling sensation when touching or pressing the area near the surgical site on the right side.
Is this situation normal? Additionally, how long after the surgery does it typically take for him to fully recover and regain fertility function? Thank you.
A-Nuo, 30~39 year old female. Ask Date: 2016/08/10
Dr. Fu Shizhang reply Urology
Postoperative pressure pain is a normal condition and is not related to sperm quality.
If fertility is desired, it is recommended to check sperm quality.
Generally, improvement is expected three months after the surgery.

Post-varicocele surgery recovery and fertility concerns are important topics for couples trying to conceive. Varicocele, which is an enlargement of the veins within the scrotum, can affect sperm production and quality, leading to fertility issues. Surgical intervention is often recommended to alleviate these problems, and understanding the recovery process is crucial for both the patient and their partner.
In your case, it’s been nearly six months since your husband's varicocele surgery, and he is experiencing a tingling sensation near the surgical site. This sensation can be relatively common during the recovery phase. After varicocele surgery, it is not unusual for patients to experience some residual discomfort, numbness, or tingling in the area due to nerve irritation or healing tissues. However, if the sensation persists or worsens, it would be wise to consult with the surgeon or a urologist to rule out any complications such as nerve entrapment or infection.
Regarding the timeline for recovery and fertility restoration, most men can return to normal activities within a few weeks post-surgery. However, the full recovery of sperm parameters can take longer. It is generally advised to wait at least three months after surgery before expecting significant improvements in sperm quality, as sperm production takes about 72 days. Therefore, it may take up to six months or more for the body to fully recover and for sperm parameters to normalize.
It’s also important to consider that while surgery can improve fertility, it does not guarantee conception. Factors such as the female partner's reproductive health, timing of intercourse, and overall lifestyle can also play significant roles in achieving pregnancy. If you and your husband have been trying to conceive for several months post-surgery without success, it may be beneficial to consult a fertility specialist. They can conduct a comprehensive evaluation of both partners to identify any additional factors that may be affecting fertility.
In terms of lifestyle modifications, maintaining a healthy diet, managing stress, and avoiding substances that can negatively impact fertility (such as tobacco and excessive alcohol) can also be beneficial. Regular follow-ups with the healthcare provider are essential to monitor recovery and address any concerns that may arise.
In summary, the tingling sensation your husband is experiencing may be a normal part of the recovery process, but it should be monitored. Full recovery and improvement in fertility can take several months, and if conception does not occur after a reasonable period, seeking further evaluation from a fertility specialist may be the next step. It’s important to maintain open communication with each other and your healthcare providers throughout this journey.
