Gynecomastia

Hello, my nipples change size due to temperature fluctuations, which has been bothering me for a long time.
About a month ago, I visited a physician named Dr.
Liao at the Tri-Service General Hospital.
He diagnosed me with gynecomastia and performed surgery to remove the subcutaneous breast tissue.
However, my issue remains unresolved, and I didn't expect that the surgery would not help.
I would like to know if there are other possible causes.

josefu, 20~29 year old female. Ask Date: 2016/08/13

Dr. Lu Daokuan reply Plastic Surgery

The nipples have nerve endings, so it is not surprising that they can expand and contract with temperature changes.
After a mastectomy, this phenomenon can still be quite noticeable, and you may consult your original surgeon for a nipple reduction procedure.

More Info

Gynecomastia, commonly referred to as male breast enlargement, is a condition that can cause significant psychological distress for those affected. It is characterized by the enlargement of breast tissue in males, which can occur due to hormonal imbalances, certain medications, or underlying health conditions. Understanding the causes and exploring alternative solutions is crucial for managing this condition effectively.

Causes of Gynecomastia
1. Hormonal Imbalance: The most common cause of gynecomastia is an imbalance between testosterone and estrogen levels in the body. While testosterone is the primary male hormone, estrogen is present in lower levels. An increase in estrogen or a decrease in testosterone can lead to breast tissue growth.

2. Puberty: Many boys experience gynecomastia during puberty due to natural hormonal changes. This condition often resolves on its own as hormone levels stabilize.

3. Aging: As men age, testosterone levels naturally decline, which can lead to an increase in breast tissue.

4. Medications: Certain medications can contribute to gynecomastia. These include anti-androgens (used for prostate cancer), anabolic steroids, some antidepressants, and medications for heart conditions.

5. Health Conditions: Conditions such as liver disease, kidney disease, hyperthyroidism, and tumors affecting the pituitary gland can disrupt hormone levels and lead to gynecomastia.

6. Substance Use: Alcohol, marijuana, and other drugs can also contribute to the development of gynecomastia.

Alternative Solutions
If surgical intervention has not resolved your symptoms, there are several alternative approaches you might consider:
1. Lifestyle Changes: Weight loss can sometimes reduce breast tissue size, particularly if the gynecomastia is related to obesity. A balanced diet and regular exercise can help manage weight and hormone levels.

2. Medication Review: If you are taking medications that may contribute to gynecomastia, consult with your healthcare provider. They may be able to adjust your medications or suggest alternatives that do not have this side effect.

3. Hormonal Therapy: In some cases, medications that adjust hormone levels, such as selective estrogen receptor modulators (SERMs) or aromatase inhibitors, may be prescribed to help reduce breast tissue.

4. Psychological Support: Counseling or support groups can be beneficial for coping with the emotional aspects of gynecomastia. Speaking with a mental health professional can help address any anxiety or self-esteem issues related to the condition.

5. Natural Remedies: Some individuals explore herbal supplements or natural remedies, although these should be approached with caution. Always consult with a healthcare professional before starting any new treatment.

6. Regular Monitoring: If gynecomastia is not causing significant discomfort or psychological distress, regular monitoring may be an option. In some cases, the condition may resolve on its own over time.

Conclusion
Gynecomastia can be a challenging condition, especially when surgical options do not yield the desired results. Understanding the underlying causes is essential for effective management. If you continue to experience symptoms, it is advisable to seek a second opinion or consult with an endocrinologist who specializes in hormonal disorders. They can provide a comprehensive evaluation and tailor a treatment plan that addresses your specific situation. Remember, you are not alone in this, and there are various avenues to explore for relief and management of gynecomastia.
