There is a thumping sound in my left ear, accompanied by itching and a feeling of fullness?
Hello, doctor.
I would like to ask about a recent issue I've been experiencing.
For the past few days, I've been hearing a faint thumping sound in my left ear, accompanied by a slight itching and a feeling of fullness.
What could be the cause of this? Since I won't be able to see a doctor until after September, are there any ways to improve this condition? Thank you.
Xiongshi, 20~29 year old female. Ask Date: 2016/08/15

Hello, the issues you mentioned may be due to 1) Eustachian tube dysfunction/tinnitus 2) External ear canal inflammation +/- earwax impaction 3) Otitis media: a) Acute otitis media: often occurs alongside upper respiratory infections (colds), sinusitis, etc., and in severe cases, there may be ear discharge or pus b) Chronic otitis media with acute exacerbation: usually occurs when there is a pre-existing perforation in the eardrum, which can be caused by unclean water or improper ear cleaning, leading to inflammation.
All of the above conditions can be appropriately managed with medication through medical consultation.
It is important to note that if there is associated hearing loss, sudden sensorineural hearing loss must be ruled out.
I recommend undergoing a pure tone audiometry test at a hospital (you will need to wear headphones in a soundproof room, where sounds will be presented and you should press a button, rather than just using a tuning fork test).
If you have associated hearing loss but have not undergone a pure tone audiometry test, it is advisable to seek medical evaluation to rule out sudden hearing loss.
The golden treatment window for sudden hearing loss is two weeks, and early intervention yields better outcomes.
If you do not have associated hearing loss and only experience a mild thumping sound, it is more likely due to Eustachian tube dysfunction/tinnitus.
During episodes, swallowing may help alleviate symptoms, and it is advisable to avoid noise and not use headphones to listen to music.
However, the safest approach is to seek medical treatment as soon as possible.
Wishing you peace and good health.
Sincerely,
Dr.
Chi Chao-Yun,
Director of the Department of Otolaryngology, Taichung Hospital, Ministry of Health and Welfare.

The symptoms you are experiencing in your left ear, specifically the thumping sound and the itching sensation, can be attributed to several potential causes. Understanding these causes can help in determining the appropriate course of action for relief.
1. Eustachian Tube Dysfunction: One of the most common reasons for a thumping sound in the ear is Eustachian tube dysfunction. The Eustachian tube connects the middle ear to the back of the nose and helps equalize pressure. If this tube is not functioning properly, it can lead to a sensation of fullness, pressure changes, and even sounds like thumping or popping. This dysfunction can be exacerbated by allergies, colds, or sinus infections, which may also explain the itching sensation as these conditions can cause inflammation in the nasal passages and Eustachian tube.
2. Earwax Buildup: Another possible cause is the accumulation of earwax (cerumen). When earwax builds up, it can create pressure in the ear and lead to sounds like thumping or popping. Itching can also occur if the ear canal becomes irritated due to excessive earwax or if the wax is pushing against the eardrum.
3. Middle Ear Issues: Conditions such as otitis media (middle ear infection) can cause similar symptoms. While you mentioned that you do not have significant pain or hearing loss, mild infections can sometimes present with thumping sounds and a feeling of fullness in the ear. Itching may also occur if there is irritation in the ear canal.
4. Tinnitus: Tinnitus is a condition characterized by hearing sounds that are not present in the environment, such as ringing, buzzing, or thumping. It can be caused by various factors, including exposure to loud noises, ear infections, or even stress. The itching sensation could be a separate issue or related to the same underlying cause.
5. Allergic Reactions: Given your history of allergic rhinitis, it is possible that allergies are contributing to your symptoms. Allergies can lead to inflammation in the ear and nasal passages, causing sensations of itching and pressure.
Recommendations for Relief
1. Nasal Decongestants: If you suspect that Eustachian tube dysfunction is the cause, over-the-counter nasal decongestants or antihistamines may help reduce nasal congestion and improve Eustachian tube function.
2. Earwax Removal: If you suspect earwax buildup, you can try using over-the-counter ear drops designed to soften earwax. However, avoid using cotton swabs, as they can push wax further into the ear canal.
3. Warm Compress: Applying a warm compress to the affected ear may help alleviate discomfort and promote drainage if there is any fluid buildup.
4. Stay Hydrated: Drinking plenty of fluids can help thin mucus and promote drainage from the Eustachian tubes.
5. Avoid Irritants: If allergies are a factor, try to avoid known allergens and consider using an air purifier to reduce indoor allergens.
6. Consult a Specialist: Since you mentioned that you have not been able to see a doctor, it is advisable to schedule an appointment with an ear, nose, and throat (ENT) specialist as soon as possible. They can perform a thorough examination, including hearing tests and possibly imaging studies, to determine the exact cause of your symptoms and recommend appropriate treatment.
In conclusion, while there are several potential causes for the thumping and itching sensations in your left ear, addressing underlying issues such as Eustachian tube dysfunction, earwax buildup, or allergies can often provide relief. It is important to seek medical advice for a proper diagnosis and treatment plan, especially if symptoms persist or worsen.
