Abnormal Vaginal Bleeding: Causes and Treatments - Obstetrics and Gynecology

Vaginal bleeding

I have been experiencing chronic vaginal inflammation and cervical erosion, which keeps recurring.
I have undergone Pap smears and HPV tests, and the results showed inflammation without any cancer cells detected.
A few days ago, I had a fungal infection, and although I felt better during the treatment, I suddenly experienced bleeding yesterday, even though my menstrual period is not due yet.
Typically, cervical erosion only bleeds during sexual intercourse, but I haven't had any, so why am I bleeding out of the blue? It's really frustrating that I can't seem to get better.
Can I undergo cryotherapy or electrocautery? Will these procedures provide a permanent solution?

Dr. Huang Yiwen reply Obstetrics and Gynecology

Hello, Miss Anonymous: Cervical erosion or inflammation may not always cause bleeding only during sexual intercourse; it can also occur spontaneously at times.
If you experience recurrent bleeding, it is advisable to consider cryotherapy or electrosurgical treatment for the cervix.
However, after treatment, continuous monitoring is necessary to check for recurrence, as it is not guaranteed to be a permanent solution.

Abnormal vaginal bleeding can be a distressing issue for many women, and understanding its causes and potential treatments is crucial for managing this condition effectively. Based on your description, it seems you have been dealing with recurrent vaginal inflammation and cervical erosion, which can contribute to abnormal bleeding. Let's delve into the possible causes, treatments, and considerations regarding your situation.

Causes of Abnormal Vaginal Bleeding
1. Cervical Erosion: Cervical erosion, or cervical ectropion, occurs when the cells from the inside of the cervical canal grow on the outside of the cervix. This condition can lead to increased sensitivity and bleeding, especially after intercourse or during a gynecological examination. However, it can also bleed spontaneously, as you have experienced.

2. Infections: Vaginal infections, including yeast infections and bacterial vaginosis, can cause inflammation and irritation of the vaginal walls, leading to abnormal bleeding. The presence of a fungal infection, as you mentioned, could be a contributing factor.

3. Hormonal Imbalances: Fluctuations in hormone levels can lead to irregular bleeding. Conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders can disrupt the menstrual cycle and cause unexpected bleeding.

4. Cervical or Uterine Polyps: These benign growths can develop on the cervix or inside the uterus and may cause irregular bleeding. While your recent tests did not indicate cancer, polyps can still be a source of bleeding.

5. Endometrial Issues: Conditions affecting the endometrium, such as endometrial hyperplasia or fibroids, can also lead to abnormal bleeding. Although your recent tests may have ruled out some serious conditions, further evaluation might be necessary if bleeding persists.

Treatment Options
1. Medical Management: If your bleeding is related to inflammation or infection, your healthcare provider may prescribe antibiotics or antifungal medications to address the underlying cause. Hormonal treatments, such as birth control pills, can help regulate your menstrual cycle and reduce the frequency of abnormal bleeding.

2. Surgical Options: If conservative treatments fail and your symptoms persist, procedures like cryotherapy (freezing) or cauterization (burning) may be considered. These methods can help remove abnormal tissue and reduce the likelihood of future bleeding. However, it's essential to discuss the risks and benefits of these procedures with your healthcare provider.

3. Regular Monitoring: Since you have a history of cervical issues, regular Pap smears and HPV testing are crucial for monitoring your cervical health. Staying on top of these screenings can help catch any changes early.

4. Lifestyle Modifications: Maintaining a healthy lifestyle, including a balanced diet, regular exercise, and stress management, can positively impact your hormonal balance and overall reproductive health.

Conclusion
Your frustration with recurrent issues is understandable, and it's essential to communicate openly with your healthcare provider about your symptoms and concerns. If you feel that your current treatment plan is not effective, consider seeking a second opinion or discussing alternative treatment options. Remember, while procedures like cryotherapy or cauterization can be effective, they are not guaranteed to be a one-time solution. Continuous follow-up and monitoring are vital for managing your reproductive health effectively.
If you have further questions or concerns, do not hesitate to reach out to your healthcare provider for personalized advice and support.
