Redness and swelling of the urethral opening?
Hello, Doctor.
I would like to ask why every time I finish ejaculation (DIY), my urethral opening appears as shown in the following image: http://imgur.com/a/AhHqg.
Sometimes, I also experience a feeling of pressure and pain inside my anus.
After ejaculation, it looks like the image above, but after about five hours, the urethral opening returns to normal.
This situation has been occurring for several years (3-4 years).
After ejaculation, I also experience back pain in the morning.
I previously consulted a urologist and underwent a urinalysis, which was reported as normal.
However, the urinalysis was not conducted when the urethral opening was red and swollen, as I had not ejaculated at that time.
I was prescribed antibiotics, but they did not help! I have a few questions for the doctor:
1.
Could this be prostatitis? If it is prostatitis, can it cause redness and swelling of the urethral opening after ejaculation?
2.
Since the urinalysis was normal, should I ask the doctor to perform a bacterial culture?
3.
Would not mentioning the redness and swelling after ejaculation during the consultation affect the doctor's judgment?
4.
What does the doctor think this condition is? Is there an infection present?
5.
Lastly, if I make an appointment, what additional tests should I request from the doctor?
I apologize for the many questions, and I appreciate the doctor's response in advance!

When seeking medical attention, please do not hold back any information from your physician.
If you are experiencing redness and swelling at the urethral opening, a thorough examination can help identify the underlying cause.
Prostatitis and issues at the urethral opening are somewhat distinct, so the likelihood of them being related is low.
Additionally, I am not personally involved in your treatment and cannot instruct other physicians on what tests to arrange.
Ultimately, medicine is a profession that requires respect for each physician's clinical judgment.
As long as you provide an accurate account of your symptoms, I believe the cause can be identified.

Understanding urethral redness after ejaculation can be a complex issue, and it’s important to consider various potential causes and underlying conditions. Based on your description, it seems that you have been experiencing redness at the urethral opening after ejaculation for several years, along with occasional discomfort in the anal area and back pain. Here’s a comprehensive look at your concerns:
1. Prostatitis: One of the potential causes of your symptoms could be prostatitis, which is inflammation of the prostate gland. Prostatitis can lead to various symptoms, including pain during or after ejaculation, discomfort in the pelvic area, and changes in urinary function. The inflammation can cause the urethra to become irritated, leading to redness or swelling at the urethral opening. If you suspect prostatitis, it would be advisable to discuss this possibility with your healthcare provider, as they may recommend further evaluation or treatment options.
2. Infection: While your urine tests have come back normal, it’s still possible to have a localized infection that may not be detected through standard urine analysis. A culture of the urine or a specific test for sexually transmitted infections (STIs) may be warranted, especially if there are any additional risk factors or symptoms present. If you have not had a culture done, it might be worth discussing with your doctor.
3. Impact of Symptoms on Diagnosis: It’s crucial to communicate all symptoms to your healthcare provider, including the redness after ejaculation. If this symptom was not mentioned during your previous visits, it could indeed affect the physician's ability to make a comprehensive assessment. Symptoms can provide vital clues to the underlying condition, and omitting them may lead to a misdiagnosis or missed diagnosis.
4. Possible Conditions: The symptoms you describe could be indicative of several conditions, including:
- Urethritis: Inflammation of the urethra, often due to infection.
- Prostatitis: As mentioned, inflammation of the prostate can lead to similar symptoms.
- Pelvic Floor Dysfunction: Sometimes, muscle tension or spasms in the pelvic floor can lead to discomfort and symptoms after ejaculation.
- Allergic Reactions or Irritation: Certain soaps, lubricants, or even laundry detergents can cause irritation in sensitive areas.
5. Further Evaluation: If you decide to seek further medical advice, consider asking your doctor about:
- Urethral Swab or Culture: To check for specific infections that may not show up in standard urine tests.
- Transrectal Ultrasound: This can help visualize the prostate and surrounding structures.
- Cystoscopy: A procedure that allows direct visualization of the urethra and bladder, which can help identify any abnormalities.
- MRI or CT Scan: If there are concerns about other underlying issues, imaging studies may be helpful.
In conclusion, while your symptoms may not indicate a severe condition, they warrant further investigation to rule out any underlying issues. It’s essential to have an open dialogue with your healthcare provider about all your symptoms, including the redness after ejaculation, to ensure a thorough evaluation and appropriate management. If you continue to experience these symptoms, don’t hesitate to seek a second opinion or ask for a referral to a specialist, such as a urologist, who can provide more targeted care.
