Menstrual issues
Hello Doctor, my last menstrual period started on July 16, and it was expected to arrive on August 13 this month.
However, it is now 9 days late.
I had unprotected intercourse during my fertile window, but I have been staying up until 7 or 8 in the morning for the past few days.
Additionally, I have been experiencing breast engorgement for over 3 days.
I usually have breast engorgement before my period, but this time it has occurred without my period arriving.
I am worried that something might be wrong, or could it be due to my irregular sleep schedule?
Ting, 20~29 year old female. Ask Date: 2016/08/22

If pregnancy is suspected and menstruation is delayed for more than a week beyond the usual cycle without confirmation of pregnancy, it is recommended to visit a healthcare facility for examination or to consider menstrual induction.

Menstrual irregularities can be concerning, especially when they deviate from your normal cycle. In your case, you mentioned that your last menstrual period started on July 16, and you were expecting it to arrive on August 13, but it has now been delayed by nine days. Additionally, you noted that you have experienced breast engorgement for over three days, which is unusual for you.
There are several potential causes for menstrual irregularities, and understanding them can help alleviate some of your concerns. Here are some common factors that can contribute to changes in your menstrual cycle:
1. Hormonal Imbalances: The menstrual cycle is regulated by a complex interplay of hormones, including estrogen and progesterone. Any disruption in these hormone levels can lead to irregular periods. Factors such as stress, significant weight changes, or underlying health conditions can affect hormone production.
2. Stress and Lifestyle Factors: You mentioned that you have been going to bed late, which can significantly impact your body's hormonal balance. Lack of sleep and high-stress levels can lead to changes in your menstrual cycle. The body requires a certain level of rest and stability to maintain regular hormonal functions.
3. Pregnancy: Given that you have had unprotected intercourse during your fertile window, it is essential to consider the possibility of pregnancy. Breast tenderness and engorgement can be early signs of pregnancy. If you suspect this might be the case, taking a home pregnancy test could provide clarity.
4. Medical Conditions: Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or other reproductive health issues can lead to irregular periods. If your menstrual irregularities persist, it may be worth consulting a healthcare provider for further evaluation.
5. Age and Life Stage: If you are in your late teens or early twenties, your body may still be adjusting to the hormonal changes that come with menstruation. Irregularities can be more common during this time. Conversely, if you are approaching perimenopause, hormonal fluctuations can also lead to irregular cycles.
Given your symptoms and concerns, it would be advisable to monitor your menstrual cycle closely over the next few weeks. If your period does not start soon, or if you continue to experience breast engorgement without menstruation, it would be prudent to consult a healthcare professional. They can perform a thorough evaluation, including a physical examination and possibly blood tests to check hormone levels, and rule out any underlying conditions.
In summary, while lifestyle factors such as sleep patterns and stress can certainly influence your menstrual cycle, it is essential to consider other possibilities, including pregnancy and hormonal imbalances. Keeping track of your symptoms and seeking medical advice if your irregularities persist will help ensure that any potential issues are addressed promptly.
