Can I get pregnant?
Hello doctor, my menstrual period started on August 4 and ended on August 10.
On August 16, while showering with my boyfriend, we engaged in some sexual activity without a condom.
He withdrew before ejaculation, and we rinsed afterward before re-entering, but there was no ejaculation during that time.
The ejaculation only occurred later through oral sex.
My last five menstrual cycles were 35, 44, 36, 41, and 34 days.
I understand that calculating the safe period is not very accurate, so I am quite worried that my cycle might suddenly drop below 30 days.
I would like to ask how likely it is for pre-ejaculate fluid and bulbourethral gland fluid to cause fertilization.
My boyfriend has urinated multiple times since his last ejaculation, so he should have cleared out any residual sperm, right? Overall, what do you think is the likelihood of pregnancy?

Hello, the chances of pregnancy are low, but it is best to take preventive measures beforehand.
Do not rely on the safe period.
If your period is more than a week late, it is advisable to see a gynecologist.
Wishing you good health.

Understanding the risks of conception during sexual activity, especially when using withdrawal methods or engaging in activities that may involve pre-ejaculate fluid, is crucial for anyone who is sexually active. In your case, you are concerned about the possibility of conception occurring from pre-ejaculate fluid during a specific encounter with your boyfriend.
Pre-ejaculate, or pre-cum, is a fluid released by the Cowper's glands during sexual arousal. This fluid can contain sperm, although typically in much lower concentrations than ejaculate. The presence of sperm in pre-ejaculate is a subject of debate; some studies suggest that it can contain viable sperm, especially if there has been a recent ejaculation, while others indicate that it may not contain sperm at all. However, the risk of conception from pre-ejaculate is not negligible, particularly if there is any sperm present.
In your situation, you mentioned that your boyfriend had urinated multiple times before the encounter, which can help reduce the likelihood of sperm being present in the pre-ejaculate. Urination can flush out any residual sperm from the urethra, but it does not guarantee that all sperm are eliminated. Therefore, while the risk may be lower, it is not entirely absent.
Regarding your menstrual cycle, you have noted that your cycles vary between 34 to 44 days. This irregularity can make it challenging to predict ovulation accurately, which is crucial for understanding your fertile window. The fertile window typically occurs around the time of ovulation, which can vary significantly from cycle to cycle, especially in individuals with irregular cycles. If your cycle were to shorten unexpectedly, it could place you at risk for conception if you engage in sexual activity during that time.
To assess the overall risk of conception from the encounter you described, several factors come into play:
1. Timing of Ovulation: If you were close to your ovulation period during the encounter, the risk of conception would be higher. Ovulation typically occurs about 14 days before the start of your next period, but this can vary based on your cycle length.
2. Presence of Sperm: If pre-ejaculate contained viable sperm, there is a possibility of conception, especially if it coincided with your fertile window.
3. Withdrawal Method: The effectiveness of the withdrawal method relies heavily on timing and control. Even with careful withdrawal before ejaculation, there is still a risk of sperm being present in pre-ejaculate.
Given these considerations, while the risk of conception from pre-ejaculate is lower than from full ejaculation, it is not zero. If you are concerned about the possibility of being pregnant, consider taking a home pregnancy test if your period is late or consult with a healthcare provider for further advice.
In summary, while the likelihood of conception from pre-ejaculate is generally lower than from ejaculate, it is still a possibility, especially if you are in or near your fertile window. If you are looking to avoid pregnancy, consider discussing more reliable contraceptive methods with your healthcare provider. Additionally, tracking your menstrual cycle more closely and understanding your ovulation patterns can help you make informed decisions about sexual activity and conception risks.
