Vaginal discharge has not improved?

Hello: I have been experiencing yellowish vaginal discharge for the past two to three years and have seen doctors about it.
Over the past year, I have had intimate relations with my boyfriend, but after each encounter, the discharge becomes even more yellow and increases in volume.
I have been seeing doctors for quite a while, but the issue has not been resolved.
After intercourse, I often notice that his semen has an unusual odor, but my boyfriend does not perceive anything abnormal.
Is this normal? If the male partner has a urinary tract infection, what symptoms might he exhibit?

Xiao Jing, 20~29 year old female. Ask Date: 2002/10/04

Dr. Liu Jianting reply Urology

It's really a big headache! Have you consulted a gynecologist for a thorough examination, including a pelvic exam? Male semen does have a certain odor; how unusual is it? How do you compare it? If a male has a urinary tract infection, there may be symptoms like painful urination or discharge, but there can also be asymptomatic cases.
I recommend that you see a gynecologist for a proper check-up.
Similarly, for males, it's advisable to consult a urologist.

More Info

Persistent yellow discharge can be a concerning symptom, and it’s important to understand its potential causes and appropriate solutions. Yellow discharge, particularly if it is accompanied by other symptoms, can indicate an underlying issue that may require medical attention.

Understanding Yellow Discharge
Yellow discharge can arise from various causes, including infections, hormonal changes, or even dietary factors. In women, the most common causes of yellow discharge include:
1. Infections:
- Bacterial Vaginosis (BV): This is an imbalance of the normal bacteria in the vagina, leading to an increase in certain bacteria. Symptoms may include a fishy odor and a yellowish discharge.

- Sexually Transmitted Infections (STIs): Infections such as gonorrhea or chlamydia can cause yellow or greenish discharge, often accompanied by other symptoms like pain during urination or intercourse.

- Yeast Infections: While typically associated with a thick, white discharge, some women may experience yellowish discharge, especially if there is irritation or inflammation.

2. Hormonal Changes: Fluctuations in hormones, particularly during the menstrual cycle, can lead to changes in discharge color and consistency.
3. Dietary Factors: Certain foods can affect the color of bodily fluids, including discharge. For instance, a diet high in beta-carotene (found in carrots and sweet potatoes) can lead to a yellowish tint in bodily fluids.

Symptoms to Monitor
In your case, the persistent yellow discharge, especially after intimate relations, could suggest an infection or another underlying issue. The fact that you notice an increase in discharge after intimacy, along with your boyfriend experiencing unusual semen odor, raises the possibility of a sexually transmitted infection or another type of infection affecting both partners.

What to Do Next
1. Consult a Healthcare Provider: It’s crucial to see a healthcare provider for a thorough examination. They may perform tests such as a pelvic exam, swabs for STIs, or other laboratory tests to determine the cause of the yellow discharge.

2. Partner Evaluation: If an infection is diagnosed, it’s important for both partners to be evaluated and treated to prevent reinfection. Men may not always show symptoms of infections, but they can still carry and transmit them.

3. Treatment Options: Treatment will depend on the underlying cause. For bacterial infections, antibiotics may be prescribed. If a yeast infection is diagnosed, antifungal medications may be recommended.
4. Hygiene Practices: Maintaining good hygiene can help manage symptoms. This includes wearing breathable cotton underwear, avoiding douching, and using unscented products.

5. Dietary Adjustments: If dietary factors are suspected, consider reducing the intake of foods that may contribute to the yellow discharge.

Conclusion
Persistent yellow discharge is not something to ignore, especially when it has been ongoing for years. It’s essential to seek medical advice to identify the cause and receive appropriate treatment. Both partners should be involved in the evaluation process to ensure that any potential infections are addressed effectively. Remember, open communication with your healthcare provider about your symptoms and concerns is key to finding a resolution.
