Oral isotretinoin and other medications?

Hello, doctor.
I previously consulted a physician regarding my acne issues and have been regularly taking oral isotretinoin (Roaccutane) 20mg.
Recently, I visited the doctor for my long-standing issue of thinning hair on both sides of my forehead (which was present before taking isotretinoin).
The doctor prescribed finasteride for me.
I would like to ask if these two medications can be taken on the same day, in the morning and evening? Thank you.

Niming, 20~29 year old female. Ask Date: 2016/09/26

Dr. Chen Yuemei reply Pharmacology

Hello: The Roaccutane 20mg (active ingredient: Isotretinoin) and Finta (active ingredient: Finasteride) you are taking have been reviewed in the literature, and no interactions between the two have been documented.

When considering the simultaneous use of oral isotretinoin (commonly known as Roaccutane) and finasteride, it is essential to understand the pharmacological properties and potential interactions of these medications.
Oral isotretinoin is primarily used for the treatment of severe acne, particularly nodular acne that has not responded to other treatments. It works by significantly reducing sebaceous gland activity, leading to decreased oil production, which helps in clearing acne. On the other hand, finasteride is commonly prescribed for conditions like androgenetic alopecia (male pattern baldness) and benign prostatic hyperplasia (BPH). It functions by inhibiting the enzyme 5-alpha-reductase, which converts testosterone to dihydrotestosterone (DHT), a hormone that contributes to hair loss in genetically predisposed individuals.

From a pharmacological standpoint, there are no known direct interactions between isotretinoin and finasteride that would contraindicate their concurrent use. This means that taking both medications on the same day, spaced out as per your physician's instructions, is generally considered safe. However, it is crucial to follow the dosing recommendations provided by your healthcare provider, as both medications can have significant side effects.

Isotretinoin is known for its potential to cause various side effects, including dry skin, chapped lips, and more severe effects such as liver enzyme elevation and teratogenicity (risk of birth defects if taken during pregnancy). Therefore, it is essential to have regular follow-ups with your healthcare provider to monitor for these side effects, especially liver function tests and lipid levels.

Finasteride, while generally well-tolerated, can also have side effects, including sexual dysfunction (such as decreased libido, erectile dysfunction, and ejaculation disorders) and breast tenderness or enlargement. These side effects can be concerning for some patients, particularly those who are already experiencing anxiety related to hair loss or other issues.

Given that you are taking isotretinoin for acne and finasteride for hair loss, it is also worth noting that both conditions can be influenced by hormonal levels. Therefore, it is advisable to keep your healthcare provider informed about any new symptoms or side effects you may experience while on these medications.
In summary, taking oral isotretinoin and finasteride together is generally safe, and they can be taken on the same day with appropriate spacing. However, it is crucial to maintain open communication with your healthcare provider regarding your treatment plan and any side effects you may encounter. Regular monitoring and follow-up appointments will help ensure that both conditions are managed effectively while minimizing potential risks associated with the medications. Always consult your healthcare provider before making any changes to your medication regimen.
