Stool becomes thin?

Hello Dr.
Ke, I would like to ask about my wife.
In September of this year, she had a fecal occult blood test that showed positive results, but it coincided with her menstrual period.
I'm wondering if that could affect the test results.
Additionally, she has daily bowel movements, but she mentions that her stools are thin.
Could this be a symptom of colorectal cancer? Thank you.

A Niu, 40~49 year old female. Ask Date: 2016/09/19

Dr. Ke Fangxu reply Surgery

Indeed, menstruation can potentially contaminate stool samples, so it is advisable to find a suitable time to retest for fecal occult blood.
Most instances of fine stool are caused by hemorrhoids; however, the possibility of malignancy cannot be completely ruled out.
If the fecal occult blood test is positive and there are abnormal stool characteristics, it is recommended to undergo a colonoscopy for further evaluation.

Understanding thin stools and their potential implications is crucial, especially when there are concerning symptoms such as the presence of blood in the stool. In your wife's case, the fact that she had a positive fecal occult blood test (FOBT) during her menstrual period could indeed affect the results. Menstrual blood can lead to false positives in stool tests, so it is essential to consider this factor when interpreting the results. If the test was conducted during her period, it may be advisable to repeat the test after her cycle has ended to ensure accuracy.

Regarding the observation of thin stools, it is important to note that changes in stool shape can be indicative of various gastrointestinal issues, including potential malignancies such as colon cancer. Thin or pencil-like stools can occur when there is a narrowing of the intestinal lumen, which may be caused by a mass or obstruction in the colon. However, it is essential to understand that thin stools alone are not definitive indicators of colon cancer. Other factors, such as the presence of abdominal pain, changes in bowel habits, weight loss, or blood in the stool, should also be considered.

Colon cancer typically presents with a range of symptoms, and while thin stools can be a sign, they are not exclusive to cancer. Other benign conditions, such as irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), or even dietary changes, can also lead to alterations in stool consistency and shape. Therefore, it is crucial to evaluate the complete clinical picture rather than focusing solely on one symptom.

Given your wife's situation, it would be prudent to consult with a healthcare professional, preferably a gastroenterologist, who can conduct a thorough evaluation. This may include a detailed medical history, physical examination, and possibly further diagnostic tests such as a colonoscopy. A colonoscopy is particularly important as it allows direct visualization of the colon and rectum and can help identify any abnormalities, such as polyps or tumors, that may require further investigation or intervention.

In summary, while thin stools and a positive fecal occult blood test can be concerning, they are not definitive signs of colon cancer on their own. It is essential to consider the context of these symptoms and seek further evaluation to determine the underlying cause. Early detection and intervention are key in managing any potential gastrointestinal issues, so addressing these concerns with a healthcare provider is the best course of action.
