Dizziness, nausea, chest tightness, and palpitations?

Hello doctor, it started with dizziness and a feeling of almost fainting.
After a while, I began to feel nauseous.
About 1 to 2 hours later, I started to feel chest tightness, as if my chest was being compressed, along with some difficulty breathing and brief chest pain.
After sleeping for a while, I woke up feeling my heart racing; I measured it several times and it was around 90 beats per minute, with some pauses in between.
After resting, the dizziness and nausea persisted, and the chest tightness did not improve.
I was previously diagnosed with mitral valve prolapse.
Are these symptoms related to that? Thank you.

Dr. He Dongjin reply Cardiology

Hello, these symptoms may be related to mitral valve prolapse and autonomic nervous system dysfunction.
The recent change in seasons could also contribute to feelings of dizziness.
It is advisable to monitor the situation.
If discomfort persists, it is recommended to visit an outpatient clinic for further examination.

More Info

Understanding symptoms such as dizziness, nausea, chest tightness, and palpitations can be complex, especially when there is a history of heart conditions like mitral valve prolapse. Your symptoms, which include dizziness, a sensation of near-fainting, nausea, chest tightness, and palpitations, warrant careful consideration and evaluation.

Dizziness and Nausea
Dizziness can be caused by various factors, including dehydration, anxiety, inner ear issues, or cardiovascular problems. Nausea often accompanies dizziness, particularly if the dizziness is severe or if it is related to anxiety. In some cases, it can also be a sign of gastrointestinal issues or even a reaction to medications.

Chest Tightness and Palpitations
Chest tightness and the feeling of pressure in the chest can be alarming, especially when combined with palpitations (a sensation of a rapid or irregular heartbeat). These symptoms can be associated with anxiety, panic attacks, or more serious cardiovascular issues. Given your history of mitral valve prolapse, it is essential to consider that this condition can sometimes lead to palpitations and chest discomfort. Mitral valve prolapse occurs when the valve between the heart's left atrium and left ventricle doesn't close properly, which can lead to various symptoms, including those you are experiencing.

Potential Connection to Mitral Valve Prolapse
Mitral valve prolapse can lead to a range of symptoms, including:
- Palpitations: Many individuals with mitral valve prolapse report feeling their heart racing or skipping beats.

- Chest Pain or Discomfort: This can occur due to the heart's irregular contractions or anxiety related to the condition.

- Dizziness: This may arise from changes in blood flow or heart rhythm.

- Shortness of Breath: This can occur during exertion or even at rest, particularly if there is associated mitral regurgitation (leakage of blood backward through the valve).

Other Considerations
While your symptoms may be related to mitral valve prolapse, they could also indicate other conditions, such as:
- Anxiety or Panic Disorders: These can manifest with physical symptoms that mimic cardiac issues.

- Cardiac Arrhythmias: Irregular heartbeats can cause dizziness and palpitations.

- Gastroesophageal Reflux Disease (GERD): This can cause chest discomfort and nausea.

- Other Cardiac Conditions: Conditions such as coronary artery disease or pericarditis should also be ruled out.

Recommendations
1. Seek Medical Attention: Given the nature of your symptoms, it is crucial to consult with a healthcare provider. They may recommend tests such as an electrocardiogram (ECG), echocardiogram, or Holter monitor to assess your heart's function and rhythm.

2. Monitor Symptoms: Keep a record of when your symptoms occur, their duration, and any associated factors (like stress or physical activity).

3. Lifestyle Modifications: Staying hydrated, managing stress, and avoiding stimulants (like caffeine) can help mitigate some symptoms.

4. Medication Review: If you are on medications, discuss with your doctor whether they could be contributing to your symptoms.

In summary, while your symptoms may be related to mitral valve prolapse, they could also indicate other underlying issues. A thorough evaluation by a healthcare professional is essential to determine the cause and appropriate treatment. Your health and well-being are paramount, so do not hesitate to seek help.
