Issues Related to Silicone Oil Removal After Retinal Detachment Surgery
Hello, my left eye underwent scleral buckling and vitrectomy due to retinal detachment, and I have had cataract surgery with silicone oil injection.
It has been 10 months.
I would like to know: currently, when I watch TV, I see two screens, one from my left eye and one from my right eye, is this normal? Will I need to be hospitalized after the silicone oil removal surgery? Thank you.

Dr. He Jianxian reply Ophthalmology
Silicone oil removal generally needs to be performed after several years of stability, so it is best not to remove it casually at this time.
As for the issue of double vision, it may be necessary to wait until the treatment reaches a certain point of completion before considering addressing it.
This is for your reference.

Hello! Thank you for your question regarding silicone oil removal after retinal detachment surgery. It sounds like you have been through a significant journey with your eye health, and I appreciate your concerns about your current vision and the upcoming procedure.
Firstly, experiencing double vision, or diplopia, where you see two images instead of one, can occur after retinal surgery, especially if there has been a significant alteration in the eye's anatomy or if the retina has not fully healed. This can be due to several factors, including the presence of silicone oil, which can affect how light is focused in the eye. It is essential to discuss this symptom with your ophthalmologist, as they can evaluate your specific situation and determine the best course of action.
Regarding the silicone oil removal surgery, it is typically performed as an outpatient procedure, meaning that you may not need to stay overnight in the hospital. However, this can vary based on individual circumstances, such as your overall health, the complexity of the surgery, and your doctor's recommendations. Post-operative care is crucial, and your doctor will provide specific instructions on how to care for your eye after the procedure.
Silicone oil is used in retinal surgeries to help keep the retina in place, but it can also lead to complications, such as emulsification (the breakdown of the oil into smaller droplets) or increased intraocular pressure, which can contribute to glaucoma. If you have had silicone oil in your eye for an extended period, as you mentioned (10 months), it is essential to monitor for these potential issues. Your doctor will likely assess the condition of the silicone oil and your retina before deciding on the timing of the removal.
After the removal of silicone oil, many patients experience an improvement in vision, but this can vary widely. Some may notice immediate changes, while others may take time to adjust as the eye heals. It's also important to note that the presence of silicone oil can sometimes mask underlying issues, so your vision may improve further once it is removed.
In terms of your vision post-surgery, if you are still experiencing double vision or other visual disturbances, it may be necessary to explore additional treatments, such as prism glasses or further surgical interventions, depending on the cause of the symptoms.
Lastly, I encourage you to maintain open communication with your ophthalmologist. They can provide personalized advice based on your specific condition and help you understand what to expect during and after the silicone oil removal procedure. Regular follow-ups are crucial to monitor your recovery and address any concerns that may arise.
In summary, while experiencing double vision after retinal detachment surgery can be concerning, it is essential to consult with your eye care professional to determine the underlying cause and appropriate treatment. Silicone oil removal is generally an outpatient procedure, but individual circumstances may vary. Your doctor will guide you through the process and help you achieve the best possible outcome for your vision. Thank you for your question, and I wish you the best in your recovery!
