Ejaculation issues
Hello doctor, recently after I ejaculate from masturbation, I feel a cramping or stabbing pain in my left abdomen.
I have visited a urologist.
During my first visit, an ultrasound was performed, and they mentioned a small inguinal hernia with a 0.2 cm defect, advising me to continue monitoring it.
Can a hernia affect ejaculation and cause left abdominal pain? The pain persisted, so I went to another hospital where they conducted a semen analysis and found inflammation in my testicles.
They prescribed some medication and advised me to exercise more.
After a week of rest, I still experience this issue after ejaculation.
Could you please advise which part might be problematic?

Hello: Your doctor has answered your questions in detail.
Postoperative complications from inguinal hernia surgery, as well as inflammation of the testicles or prostate and neuromuscular pain, are all possible.
It is recommended to follow your doctor's treatment advice and to avoid excessive masturbation for the next few days.
Wishing you good health.

More Info
Ejaculation pain, particularly when accompanied by abdominal discomfort, can be a perplexing issue that may stem from various underlying causes. In your case, the left abdominal pain following ejaculation could be related to several factors, including the presence of a hernia, inflammation of the testicles (orchitis), or other pelvic conditions.
Possible Causes of Pain After Ejaculation
1. Hernia: You mentioned that an ultrasound indicated a small hernia (approximately 0.2 cm). While small hernias may not always cause symptoms, they can sometimes lead to discomfort, especially during activities that increase intra-abdominal pressure, such as ejaculation. The pain you experience could be related to the hernia, particularly if it is located in the inguinal region, which is common in males.
2. Orchitis: The second opinion you received indicated that there was inflammation in the testicles. Orchitis can cause pain that radiates to the abdomen, especially during ejaculation. This inflammation can be due to infections (viral or bacterial) or other inflammatory conditions. If the inflammation persists, it may contribute to ongoing discomfort.
3. Pelvic Floor Dysfunction: The pelvic floor muscles play a crucial role in sexual function and ejaculation. If these muscles are tense or dysfunctional, they can lead to pain during or after ejaculation. This is often associated with conditions like prostatitis or pelvic pain syndrome.
4. Nerve Irritation: Sometimes, nerve irritation in the pelvic region can lead to referred pain in the abdomen. Conditions such as pudendal neuralgia can cause pain during sexual activity or ejaculation.
5. Other Urological Issues: Conditions such as prostatitis (inflammation of the prostate gland) or seminal vesiculitis (inflammation of the seminal vesicles) can also lead to pain during ejaculation. These conditions may not always present with obvious symptoms but can cause discomfort.
Recommendations for Further Evaluation
Given that your symptoms have persisted despite initial evaluations and treatments, it may be beneficial to pursue further diagnostic steps:
1. Consult a Urologist: Since you have already seen a urologist, consider seeking a second opinion from another specialist who may have different insights or approaches to your condition. A thorough examination, including a digital rectal exam, may provide additional information.
2. Imaging Studies: If not already done, advanced imaging studies such as a CT scan or MRI may help visualize the pelvic structures more clearly and identify any abnormalities that may not have been apparent on ultrasound.
3. Pelvic Floor Assessment: A referral to a pelvic floor physical therapist could be beneficial. They can assess for any muscle tension or dysfunction that may be contributing to your pain.
4. Infection Workup: If not already performed, consider a comprehensive workup for infections, including sexually transmitted infections (STIs) and other urological infections.
5. Lifestyle Modifications: While waiting for further evaluations, consider lifestyle changes such as avoiding heavy lifting, managing stress, and practicing relaxation techniques, which may help alleviate some discomfort.
Conclusion
In summary, the pain you are experiencing after ejaculation could be multifactorial, involving hernia, inflammation, pelvic floor dysfunction, or nerve irritation. It is crucial to continue working with healthcare professionals to explore these possibilities and find an effective treatment plan. Persistent pain should not be ignored, and further investigation is warranted to ensure that any underlying conditions are appropriately addressed.
