Can I get pregnant?

Hello Dr.
Chang: My girlfriend and I had sexual intercourse on October 1st and 2nd without using a condom, and I did not ejaculate inside her; I pulled out and ejaculated outside after about 6 to 10 seconds.
However, there were a few times when I felt close to ejaculation but had not pulled out yet, although I did not actually ejaculate inside her.
Is there a possibility of her becoming pregnant under these circumstances?

Dear inquirer: There is a possibility of pregnancy even if ejaculation has not occurred, as pre-ejaculatory fluid may contain sperm.
It is recommended to check for pregnancy if your period is more than a week late.
Thank you! Wishing you peace and health, Dr.
Zhang.

The question of whether pregnancy can occur without ejaculation during intercourse is a common concern among couples. To address this, it's important to understand the mechanics of conception and the factors involved in pregnancy.

Firstly, pregnancy occurs when sperm fertilizes an egg. Sperm is typically released during ejaculation, but it is crucial to note that pre-ejaculatory fluid, often referred to as "pre-cum," can also contain sperm. This fluid is released before ejaculation and may carry viable sperm from a previous ejaculation if there is residual sperm in the urethra. Therefore, even if ejaculation does not occur inside the vagina, there is still a potential risk of pregnancy if pre-ejaculatory fluid comes into contact with the vaginal canal.

In your situation, you mentioned that you and your girlfriend engaged in sexual intercourse without a condom and that you were careful to withdraw before ejaculation. However, if there were instances where you felt close to ejaculation but did not withdraw in time, there is a possibility that pre-ejaculatory fluid could have been released. This fluid can potentially lead to pregnancy, albeit the risk is generally lower than with full ejaculation.

The likelihood of pregnancy in such scenarios varies based on several factors, including the timing of the woman's menstrual cycle. If intercourse occurs during her fertile window (typically around ovulation), the chances of conception increase significantly. The fertile window is usually a few days before and the day of ovulation, which occurs approximately 14 days before the start of the next menstrual period in a typical 28-day cycle.

In summary, while the risk of pregnancy without ejaculation is lower than with ejaculation, it is not zero. Pre-ejaculatory fluid can contain sperm, and if it comes into contact with the vagina, there is a possibility of pregnancy. If you and your girlfriend are concerned about the possibility of pregnancy, it may be wise to consider emergency contraception if the intercourse occurred within the last 72-120 hours and you wish to prevent pregnancy. Additionally, for future sexual encounters, using condoms or other forms of contraception can significantly reduce the risk of unintended pregnancy and also protect against sexually transmitted infections (STIs).

If your girlfriend misses her period or experiences any symptoms of pregnancy, it would be advisable for her to take a home pregnancy test or consult a healthcare provider for further evaluation. Regular communication about contraception and reproductive health is essential for both partners to make informed decisions regarding their sexual health.
