Impacted Wisdom Teeth: To Extract or Not? - Dentistry

Wisdom teeth and molars?

Hello Doctor, a few days ago during my dental check-up, I discovered that my wisdom teeth are horizontally impacted, but they haven't erupted and are still buried in my gums.
I don't have any tooth pain or other issues, but the dentist suggested extraction to prevent any potential impact on the growth of my other teeth.
However, the dentist also mentioned that my teeth alignment is not significantly affected by the wisdom teeth, so extraction is not strictly necessary.
I would like to ask for your advice regarding horizontally impacted wisdom teeth that have not erupted.
Additionally, I noticed that even my molars are not fully erupted; they are closer to the gums compared to the adjacent teeth, and my teeth are generally smaller (my incisors are about the size of a pinky nail).
Will my wisdom teeth eventually erupt? Thank you for your response.

Hello: Whether to extract impacted wisdom teeth depends on the following two factors: 1.
Are you able to thoroughly clean the teeth in that area? If so, and your cavity rate is low, you may consider not extracting them.
If not, it may lead to cavities in the adjacent teeth and periodontal disease.
2.
Wisdom tooth extraction is a selective procedure, and any surgery carries its own risks, complications, and potential side effects.
Please discuss this thoroughly with your treating physician before making your own decision regarding extraction.
Wishing you peace and good health.

When it comes to impacted wisdom teeth, particularly those that are horizontally positioned and have not erupted, the decision to extract them can be complex and should be made based on several factors. It's important to understand the potential implications of keeping or removing these teeth, especially since you mentioned that you do not currently experience pain or other issues.

Understanding Impacted Wisdom Teeth:
1. Potential Risks of Retaining Impacted Wisdom Teeth:
- Even if your wisdom teeth are not causing immediate problems, they can lead to complications in the future. Impacted teeth can create a breeding ground for bacteria, leading to infections, gum disease, or even cysts that can damage the surrounding bone and teeth.

- They can also exert pressure on adjacent teeth, potentially causing misalignment over time, even if your current dental arrangement appears fine.

2. Monitoring and Regular Check-ups:
- If you choose not to extract your impacted wisdom teeth, it is crucial to have regular dental check-ups. Your dentist can monitor the situation and look for any signs of complications, such as changes in the position of your other teeth or the onset of gum disease.

3. Factors Influencing Extraction Decisions:
- The decision to extract should consider your age, dental health, and the position of the wisdom teeth. Younger patients often heal better and may have fewer complications post-surgery.

- If your wisdom teeth are causing crowding or if there is a risk of future dental issues, extraction may be the best option.

4. Eruption of Wisdom Teeth:
- Regarding your question about whether your wisdom teeth will eventually erupt, it is possible but not guaranteed. Factors such as the size of your jaw, the position of the teeth, and the presence of other teeth can influence whether they will emerge. In some cases, they may remain impacted indefinitely without causing problems.

5. Consultation with an Oral Surgeon:
- If you are uncertain about the best course of action, consider consulting an oral surgeon. They can provide a more detailed assessment, possibly including imaging studies like X-rays, to evaluate the position of your wisdom teeth and the surrounding structures.

6. Personal Considerations:
- Since you mentioned that your molars are also positioned closer to the gums, it may be beneficial to discuss this with your dentist. They can evaluate whether this positioning could lead to future dental issues and whether any preventive measures should be taken.

In summary, while your current situation with your horizontally impacted wisdom teeth may not seem problematic, it is essential to remain vigilant and consult with dental professionals to make informed decisions. Regular monitoring and open communication with your dentist about any changes in your oral health will help you maintain a healthy smile. If extraction is recommended, understanding the reasons behind it can help alleviate any concerns you may have about the procedure.
