Body symptom concerns?
Hello, I would like to ask a question.
I am 23 years old, but two years ago, I gained over 30 kilograms in six months, and my weight continues to increase.
I have also started to develop hair in areas of my body where I previously did not have hair, and I am experiencing persistent acne.
My menstrual periods occur every month, but the flow is very light and lasts only three days.
Additionally, I have noticed small, raised bumps in my genital area, and I have not had any sexual intercourse.
What could these symptoms indicate? How long would treatment take?
Niming, 20~29 year old female. Ask Date: 2016/10/10
Dr. Cao Guotao reply Obstetrics and Gynecology
Hello, I'm sorry that I cannot diagnose the small, grainy growths in the described area.
If you continue to experience discomfort, I recommend seeing a gynecologist for an examination.
Wishing you good health.

It sounds like you are experiencing a combination of symptoms that could be indicative of a hormonal imbalance, particularly related to androgens (male hormones) such as testosterone. Given your age and the rapid weight gain of over 30 kilograms in a short period, along with the development of body hair in areas where you previously did not have it, persistent acne, and changes in your menstrual cycle, it is essential to consider a few potential conditions.
One possibility is Polycystic Ovary Syndrome (PCOS), which is a common endocrine disorder among women of reproductive age. PCOS is characterized by a combination of symptoms, including irregular menstrual cycles, weight gain, excessive hair growth (hirsutism), and acne. The hormonal imbalance in PCOS often leads to elevated levels of androgens, which can cause the symptoms you are describing. The presence of small growths in the genital area could also be related to hormonal changes, but it is crucial to have these evaluated by a healthcare professional.
Another potential condition to consider is Cushing's syndrome, which results from prolonged exposure to high levels of cortisol, a hormone produced by the adrenal glands. Symptoms can include rapid weight gain, changes in hair growth patterns, and skin changes. However, this condition is less common and would typically present with additional symptoms such as easy bruising, purple stretch marks, and fatigue.
Given the complexity of your symptoms, it is crucial to consult with a healthcare provider, preferably an endocrinologist or a gynecologist, who can perform a thorough evaluation. This evaluation may include blood tests to check hormone levels, imaging studies to assess the ovaries and adrenal glands, and possibly other diagnostic tests to rule out other conditions.
In terms of treatment, it will depend on the underlying cause of your symptoms. If PCOS is diagnosed, treatment options may include lifestyle changes such as diet and exercise to manage weight, hormonal contraceptives to regulate menstrual cycles and reduce acne and hair growth, and medications like metformin to improve insulin sensitivity. If Cushing's syndrome is diagnosed, treatment may involve addressing the underlying cause, which could include surgery, medication, or radiation therapy.
The duration of treatment can vary widely based on the diagnosis. For conditions like PCOS, management is often ongoing and may require lifestyle changes and medication for an extended period. Regular follow-up with your healthcare provider is essential to monitor your symptoms and adjust treatment as necessary.
In summary, your symptoms warrant a comprehensive evaluation to determine the underlying cause. Early diagnosis and appropriate management can significantly improve your quality of life and help address the symptoms you are experiencing. Please seek medical attention as soon as possible to discuss your concerns and get the appropriate care.
