Testicular issues
Doctor: Hello, I live next to Taichung Hospital.
I have been troubled by three issues for about 5 to 6 years.
I have visited Dr.
Xia several times but always hesitated to bring up my urological concerns.
Today, after seeing Dr.
Sun answer some questions so clearly for netizens, I decided to muster the courage to ask Dr.
Sun.
1.
I have been married to my wife for 15 years.
Our sexual activity is not excessive, usually just two positions, and ejaculation is normal.
However, over the past 5 to 6 years, when we engage in one particular position, I experience bright red blood during ejaculation.
I have tried to go very slowly, but I still experience bleeding.
What could be the cause?
2.
If I go a week without sexual activity, why do I experience soreness in my lower back?
3.
Occasionally, when I return to my hometown in the south to help my parents with work (doing cement work), and the weather is very hot, I notice that my urine becomes very dark and has a reddish tint.
I have checked with tissue and noticed blood as well.
My lower back also feels sore and painful.
After returning to Taichung, it takes about 2 to 3 days for everything to return to normal.
P.S.
These issues have persisted for the past 5 to 6 years without resolution.
Could you please assist me, Dr.
Sun? Should I go to Taichung Hospital for an examination? Thank you.

Dr. Sun Hongshun reply Urology
Hello: Since the persistent symptoms have lasted for 5-6 years and no examinations have been conducted, I recommend that you consult with an outpatient clinic directly.
They can arrange appropriate tests to identify the cause for further management.
Wishing you good health.

It sounds like you have been experiencing a range of urological issues that have been concerning you for several years. Let's break down your questions and concerns one by one, and I'll provide some insights based on medical knowledge.
1. Blood During Ejaculation
Experiencing blood in your semen (hematospermia) can be alarming. There are several potential causes for this condition, including:
- Infection: Infections of the prostate (prostatitis), seminal vesicles, or urethra can lead to inflammation and bleeding.
- Trauma: Any injury to the pelvic area or the reproductive organs can cause bleeding.
- Vascular Issues: Abnormal blood vessels or varicoceles (enlarged veins in the scrotum) can sometimes lead to bleeding.
- Benign Prostatic Hyperplasia (BPH): An enlarged prostate can also contribute to bleeding during ejaculation.
- Malignancy: Although less common, tumors in the prostate or other parts of the reproductive system can cause blood in the semen.
Given that this has been a recurring issue for 5-6 years, it is crucial to consult a urologist for a thorough evaluation. They may recommend tests such as a urinalysis, ultrasound, or even a prostate exam to determine the underlying cause.
2. Lower Back Pain After Abstaining from Sexual Activity
Experiencing lower back pain after a week of abstaining from sexual activity can be attributed to several factors:
- Muscle Tension: Sexual activity can relieve tension in the pelvic and lower back muscles. Prolonged abstinence may lead to muscle tightness or discomfort.
- Prostate Congestion: Some men report discomfort due to the buildup of fluid in the prostate when they do not ejaculate for an extended period.
- Postural Issues: If you are engaging in activities that strain your back, such as heavy lifting or poor posture, this could contribute to your discomfort.
If this pain persists or worsens, it would be wise to seek medical advice to rule out any serious underlying conditions.
3. Dark Urine with Blood After Heavy Labor
Dark urine, especially when accompanied by a reddish tint, can indicate dehydration, particularly if you are working in hot conditions and sweating a lot. However, the presence of blood in the urine (hematuria) is concerning and can be caused by:
- Dehydration: Concentrated urine can appear darker and may have a reddish hue if there is any bleeding.
- Urinary Tract Infection (UTI): Infections can cause both dark urine and blood.
- Kidney Stones: These can cause bleeding and pain, especially after strenuous activity.
- Other Medical Conditions: Conditions such as glomerulonephritis or tumors can also lead to hematuria.
If you notice that your urine is consistently dark or contains blood, it is essential to seek medical attention. A urinalysis and possibly imaging studies can help determine the cause.
Conclusion
Given the complexity and duration of your symptoms, I strongly recommend that you visit a urologist for a comprehensive evaluation. They can perform the necessary tests to diagnose your conditions accurately and recommend appropriate treatments. Do not hesitate to discuss all your symptoms openly with your healthcare provider; they are there to help you. Early diagnosis and treatment can significantly improve your quality of life and address any underlying health issues.
