Delayed menstruation?

Hello, doctor.
My girlfriend's period is currently 19 days late.
Her shortest menstrual cycle has been 33 days and the longest 51 days.
On Saturday (17 days late), she visited a gynecologist for a pregnancy test and a transvaginal ultrasound, both of which indicated that she is not pregnant.
The doctor prescribed medication to induce her period.
The last sexual intercourse was on October 5, and we used condoms and practiced withdrawal every time.
I would like to ask, what is the likelihood of pregnancy in this situation?

J, 20~29 year old female. Ask Date: 2016/10/17

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, after taking general menstrual induction medication, menstruation usually occurs within a week, and the chances of pregnancy are low.
It is always best to use preventive measures beforehand and not to rely on the safe period.
If menstruation does not arrive after waiting another week, it is advisable to consult a gynecologist.
Wishing you good health.

More Info

Understanding delayed menstruation can be a complex issue, especially when pregnancy is a concern. In your case, your girlfriend's menstruation has been delayed for 19 days, which is significantly longer than her usual cycle. You mentioned that her menstrual cycle typically ranges from 33 to 51 days, and she has undergone a pregnancy test and ultrasound, both of which indicated that she is not pregnant. Additionally, she was prescribed medication to induce menstruation.

Given that you have consistently used condoms and practiced withdrawal during intercourse, the likelihood of pregnancy is significantly reduced. Condoms are highly effective when used correctly, with a failure rate of about 2% with perfect use and about 18% with typical use. The withdrawal method, while not as reliable as condoms, adds an additional layer of protection. Therefore, the chances of pregnancy in your situation appear to be low.

However, there are several reasons for a delayed menstrual cycle that are not related to pregnancy. Stress, hormonal imbalances, changes in weight, excessive exercise, and underlying health conditions such as polycystic ovary syndrome (PCOS) can all contribute to irregularities in the menstrual cycle. In your girlfriend's case, it may be beneficial to consider these factors, especially if she has experienced any significant life changes or stressors recently.

The fact that she has been prescribed medication to induce menstruation suggests that her healthcare provider is taking her symptoms seriously. It is essential to follow up with the doctor if her period does not return after taking the medication or if she experiences any unusual symptoms, such as severe pain or heavy bleeding.
In terms of psychological preparation, it is understandable to feel anxious about the possibility of pregnancy, especially when the menstrual cycle is delayed. Open communication with your girlfriend about her feelings and concerns can help both of you navigate this situation more comfortably. It may also be helpful for her to speak with a healthcare provider about her menstrual irregularities to rule out any underlying health issues and to discuss any potential lifestyle changes that could help regulate her cycle.

If her period does not resume after the medication, further evaluation may be necessary. This could include blood tests to check hormone levels or imaging studies to assess the reproductive organs. Understanding the underlying cause of her delayed menstruation can provide clarity and help in managing any future concerns.

In summary, while the risk of pregnancy in your situation seems low due to the consistent use of protection, it is essential to monitor her menstrual cycle and consult with a healthcare provider if irregularities persist. Addressing any underlying health issues and maintaining open communication can help alleviate anxiety and ensure both of you are well-informed about reproductive health.
