There may be a heartbeat sound in the ear?
Hello, doctor.
Over the past month, I have noticed that when I sleep on my right side, I can hear and feel a pulsing sound in my right ear, similar to a heartbeat.
However, I do not experience this issue when sleeping on my left side or on my back.
What could be the reason for this? Thank you.

Dr. Li Jiaxuan reply Otolaryngology
It is likely vascular tinnitus; it is recommended to visit an otolaryngology clinic for examination.
Wishing you good health.

Hearing a heartbeat in your ear while sleeping, often referred to as pulsatile tinnitus, can be a concerning experience. This phenomenon occurs when you perceive a rhythmic sound that corresponds to your heartbeat, and it can be particularly noticeable when lying down or in a quiet environment. There are several potential causes for this condition, and understanding them can help you determine the best course of action.
One common cause of pulsatile tinnitus is vascular issues. The sound you hear may be due to blood flow in the vessels near your ear. When you lie down, especially on one side, the position can amplify the sound of blood moving through these vessels. This is often more pronounced in individuals with certain anatomical variations or conditions that affect blood flow, such as high blood pressure or increased blood flow due to physical exertion or anxiety.
Another possible explanation is related to the ear itself. Conditions such as Eustachian tube dysfunction can lead to a sensation of fullness in the ear and may cause sounds to be perceived differently. When you are lying on your side, the pressure changes in the ear can exacerbate this sensation, leading to the perception of a heartbeat.
In some cases, pulsatile tinnitus can be associated with more serious conditions, such as vascular tumors or abnormalities in the blood vessels. However, these are less common and typically accompanied by other symptoms. If you have not experienced any additional symptoms, such as dizziness, hearing loss, or significant changes in your overall health, it is less likely that a serious condition is the cause.
Stress and anxiety can also play a role in the perception of tinnitus. When you are stressed, your body may react in ways that heighten your awareness of bodily sensations, including sounds in your ears. Additionally, lifestyle factors such as caffeine intake, smoking, and lack of sleep can contribute to the experience of tinnitus.
Given your specific situation of only hearing this sound when lying on your right side, it may be beneficial to consider factors such as your sleeping position, any recent changes in your health, or stress levels. If this symptom persists or worsens, it is advisable to consult with an ear, nose, and throat (ENT) specialist. They can perform a thorough examination, including hearing tests and imaging studies if necessary, to rule out any underlying conditions.
In summary, hearing a heartbeat in your ear while sleeping can be attributed to various factors, including vascular issues, ear-related conditions, and lifestyle factors. While it can be alarming, many cases are benign and can be managed with lifestyle adjustments and medical evaluation if needed. If you have ongoing concerns, seeking professional medical advice is always the best course of action to ensure your health and peace of mind.
