Lower abdominal tenderness on palpation?

Hello doctor, recently I've been feeling discomfort in the lower left abdomen near the navel.
I noticed that pressing near the left groin area causes a slight aching sensation.
The discomfort in the lower left abdomen is vague and not very intense.
I visited a gynecologist and had an ultrasound, and they suggested it might be due to fatigue.
Is that true? I didn't have an internal examination, just the ultrasound, but the doctor didn't say much except that the endometrium is slightly thick.
My menstrual period is due on October 8th.

There are many causes of lower abdominal pain that need to be differentiated.
Possible causes can be categorized into four types: gynecological issues, gastrointestinal problems, urinary issues, and ligament or nerve problems.
If gynecological issues are suspected, further evaluation may include abdominal palpation, vaginal examination (which requires prior sexual experience), and ultrasound examination.
If there is no improvement, further investigation is recommended.

Left lower abdominal pain can be a perplexing symptom, as it may arise from various underlying causes. Understanding the potential reasons for this discomfort is crucial for effective diagnosis and treatment. Based on the information provided, there are several possibilities to consider.

Firstly, the left lower abdomen houses several vital organs, including parts of the intestines, the left ovary, and the uterus in females. Pain in this area can often be attributed to gastrointestinal issues, such as diverticulitis, which is the inflammation of diverticula in the colon. Given your history of discomfort and the fact that you have undergone an ultrasound, it is essential to consider whether any gastrointestinal conditions might be contributing to your symptoms. Symptoms of diverticulitis can include localized pain, changes in bowel habits, and sometimes fever.

Another potential cause of left lower abdominal pain, especially in females, is related to gynecological issues. Conditions such as ovarian cysts, endometriosis, or pelvic inflammatory disease can lead to discomfort in this region. You mentioned that your ultrasound indicated a slightly thickened endometrium, which can occur during certain phases of the menstrual cycle. While this finding alone may not be alarming, it could be worth monitoring, especially if you experience additional symptoms such as heavy menstrual bleeding, severe cramping, or irregular cycles.

Musculoskeletal issues should also be considered. Sometimes, pain in the abdominal area can stem from muscle strain or ligamentous pain, particularly if you have recently engaged in activities that require bending or lifting. This type of pain is often exacerbated by movement and can be mistaken for visceral pain. If the discomfort is more pronounced with certain movements or positions, it may indicate a musculoskeletal origin.

Additionally, stress and fatigue can manifest physically, leading to muscle tension and discomfort. If your doctor suggested that fatigue might be a contributing factor, it is essential to consider your overall lifestyle, including stress levels, sleep patterns, and physical activity. Chronic stress can lead to muscle tightness and discomfort, which may be perceived as abdominal pain.

If your symptoms persist or worsen, further evaluation may be warranted. While an ultrasound is a useful diagnostic tool, additional imaging studies, such as a CT scan, may provide more insight into any underlying issues. Blood tests can also help rule out infections or inflammatory conditions.

In summary, left lower abdominal pain can arise from various causes, including gastrointestinal, gynecological, musculoskeletal, and even psychosomatic factors. It is essential to maintain open communication with your healthcare provider, especially if your symptoms do not improve or if new symptoms develop. Keeping a symptom diary that tracks the nature of your pain, associated activities, and any other symptoms can be beneficial for your healthcare provider in determining the next steps in your evaluation and management.
