Smegma issue
I have phimosis, but I can retract the foreskin when not erect.
I have a habit of masturbation, and recently I've started to clean the smegma around the urethral opening, which is red and painful to touch.
I can successfully retract the foreskin and have noticed a ring of smegma, but when I try to push the smegma away, I feel a slight stinging and discomfort.
I have tried to remove it, but it still won't come off.
What should I do?

Hello: Clinically, it may be balanoposthitis.
It is recommended to visit a urology clinic for examination and treatment.
Regarding the issue of smegma, it is sufficient to gently retract the foreskin during bathing and rinse with warm water.
Do not be overly concerned about whether there is still debris trapped or feel the need to forcibly remove it.
Wishing you good health.

Dealing with foreskin issues, particularly phimosis (the inability to retract the foreskin), can be a sensitive and sometimes distressing experience. It’s important to approach this issue with care and understanding, as it involves both physical and psychological aspects.
From your description, it seems that you are experiencing discomfort and pain when attempting to clean the area under the foreskin, which may be due to a buildup of smegma (a combination of dead skin cells, oils, and moisture) and possibly inflammation of the glans (the head of the penis). Here are some steps and considerations to help you manage this situation effectively:
1. Gentle Cleaning: It’s crucial to maintain proper hygiene. You can start by gently cleaning the area with warm water during your showers. Avoid using harsh soaps or chemicals, as these can irritate the sensitive skin. If you notice any redness or irritation, it may be beneficial to use a mild, unscented soap.
2. Gradual Retraction: Since you mentioned that you can retract the foreskin when not erect, it’s important to do this gently and gradually. You can try to retract the foreskin a little further each day, but do not force it if you feel pain. Pain is a signal from your body that you should stop.
3. Moisturizing: Applying a gentle, hypoallergenic moisturizer or a water-based lubricant can help soften the skin and make retraction easier over time. This can also help reduce discomfort when cleaning.
4. Avoiding Irritants: If you are experiencing pain at the urethral opening, it’s essential to avoid any irritants, including certain soaps, lotions, or even tight clothing. Ensure that your underwear is breathable and not too tight.
5. Consulting a Healthcare Provider: If the pain persists or if you notice any signs of infection (such as increased redness, swelling, or discharge), it’s crucial to consult a healthcare provider. They can assess your condition and may recommend treatments such as topical steroids to reduce inflammation or, in some cases, surgical options like circumcision or preputioplasty if phimosis is severe.
6. Understanding Phimosis: It’s important to understand that phimosis is common, especially in younger males, and often resolves naturally over time. However, if it causes significant discomfort or hygiene issues, medical intervention may be necessary.
7. Psychological Support: Dealing with foreskin issues can sometimes lead to anxiety or embarrassment. It’s important to remember that these issues are common and that seeking help is a sign of strength. Speaking with a healthcare provider can provide reassurance and guidance.
8. Avoiding Self-Treatment: While it might be tempting to try various home remedies or aggressive cleaning methods, these can often lead to more irritation or injury. Stick to gentle methods and consult a professional for advice tailored to your specific situation.
In summary, maintaining proper hygiene, being gentle with retraction, and consulting a healthcare provider if issues persist are key steps in managing foreskin-related concerns. Remember, you are not alone in this, and there are effective solutions available.
