Is it sinusitis?
Hello doctor, my symptoms include discomfort and pain on one side of my nose, occasional nasal congestion, and a strange odor when I breathe in.
I have seen a doctor who performed an endoscopy and an X-ray.
The endoscopy revealed some drainage from the sinus on the right side, and the X-ray showed mild haziness, which is not severe.
The doctor diagnosed me with unilateral sinusitis, but I don't have thick yellow mucus.
Is it really sinusitis if my nose hurts and has a fishy smell? This has been going on for a while.

Hello, the common symptoms of sinusitis include nasal congestion, yellow-green nasal discharge, fever, facial pain, foul smell, and changes in smell, but not all symptoms may be present.
Diagnostic methods include endoscopy and imaging studies.
If acute sinusitis persists despite antibiotic treatment for more than two weeks, it is advisable to visit an otolaryngologist for bacterial culture.
If a "more virulent" bacteria is identified, it may be necessary to switch to a stronger antibiotic.
I recommend that you purchase a "nasal irrigator" (containing isotonic saline) and perform rinsing 1-2 times daily to enhance the treatment of sinusitis.
Additionally, avoid staying up late, refrain from smoking, and if you have diabetes, control your blood sugar levels.
If treatment exceeds 2-3 months and sinusitis remains unresolved, it is highly likely to be "chronic" sinusitis, which requires further examination, and if necessary, surgical intervention.
Based on your description, it is indeed possible that you have sinusitis, and I recommend seeking evaluation and treatment from an otolaryngologist.
Wishing you peace and good health.
Sincerely,
Dr.

Based on your symptoms and the medical evaluations you've undergone, it does seem that you may be experiencing a form of sinusitis, specifically unilateral (one-sided) sinusitis. Sinusitis is an inflammation of the sinus cavities, which can lead to a variety of symptoms, including nasal pain, pressure, and sometimes a foul odor, particularly if there is an infection present.
Your description of discomfort and pain on one side of your nose, along with nasal congestion and the perception of a strange smell, aligns with common symptoms of sinusitis. The presence of a "fishy" odor could indicate the presence of bacteria or other pathogens in the sinuses, which can occur even in the absence of thick yellow or green nasal discharge. This is particularly true in cases of chronic sinusitis, where symptoms can be more subtle and may not always include the classic signs of infection, such as purulent nasal discharge.
The findings from your endoscopy and X-ray also support the diagnosis of sinusitis. The "flowing" appearance seen during the endoscopy suggests that there may be mucus drainage from the sinus cavities, which is often a sign of inflammation or infection. The X-ray showing mild haziness can indicate fluid in the sinuses, which is another hallmark of sinusitis.
It's important to note that sinusitis can be caused by various factors, including viral infections, bacterial infections, allergies, and anatomical issues such as a deviated septum or nasal polyps. In your case, since you have been experiencing these symptoms for a while, it may be worthwhile to consider whether there are underlying factors contributing to your condition.
Here are some additional points to consider:
1. Chronic vs. Acute Sinusitis: If your symptoms have persisted for more than 12 weeks, you may be dealing with chronic sinusitis. This condition often requires a different treatment approach compared to acute sinusitis, which typically resolves within a few weeks.
2. Nasal Irrigation: Regular nasal irrigation with saline solution can help clear mucus and allergens from your nasal passages, potentially alleviating some of your symptoms.
3. Allergy Management: If allergies are contributing to your sinus issues, managing your allergies with antihistamines or nasal corticosteroids may provide relief.
4. Follow-Up Care: Since you have already seen an ENT specialist, it may be beneficial to follow up with them if your symptoms do not improve or worsen. They may recommend further imaging studies, allergy testing, or even a CT scan to get a more detailed view of your sinuses.
5. Consider Other Causes: If your symptoms persist despite treatment for sinusitis, it may be worth exploring other potential causes of your nasal pain and odor, such as dental issues, foreign bodies, or even less common conditions like nasal tumors.
In conclusion, while your symptoms are consistent with unilateral sinusitis, the absence of thick yellow discharge does not rule it out. The presence of a strange odor and the findings from your examinations suggest that further evaluation and possibly a tailored treatment plan may be necessary. It’s crucial to maintain open communication with your healthcare provider to ensure that you receive the most appropriate care for your condition.
