Prolonged erection?

Hello, doctor.
I would like to ask you about something.
The day before yesterday, while watching a video, I experienced a prolonged erection without ejaculation.
I have been feeling some discomfort in my lower abdomen, and my back is also a bit sore.
Additionally, I've noticed an increase in the volume of pre-ejaculatory fluid, and my erections seem to be more sensitive.
Is this normal?

Niming, 20~29 year old female. Ask Date: 2016/11/05

Dr. Du Shixi reply Urology

Hello: Based on your description, it seems to be temporary.
Stay active and maintain a normal diet, and you should recover within 1 to 2 days.

Prolonged erection, or priapism, is a condition that can be concerning and warrants careful consideration. It is characterized by an erection that lasts longer than four hours and occurs without sexual stimulation. While occasional prolonged erections can happen, especially in response to certain stimuli, persistent or painful erections can indicate a medical emergency.

In your case, experiencing a prolonged erection while watching a movie, especially if it is accompanied by discomfort in the lower abdomen and back, is something to pay attention to. The sensations you describe, such as increased sensitivity and a feeling of fullness or pressure, can be normal in certain contexts, particularly if there is sexual arousal involved. However, if these sensations lead to discomfort or if the erection does not subside after a reasonable time, it could be indicative of a problem.

There are several potential causes of prolonged erections, including:
1. Medications: Certain medications, particularly those used to treat erectile dysfunction (like PDE5 inhibitors), can lead to prolonged erections if not taken as prescribed. Other medications, such as antidepressants or antipsychotics, may also have side effects that affect sexual function.

2. Underlying Health Conditions: Conditions such as sickle cell disease, leukemia, or other blood disorders can lead to priapism due to blood flow issues. Neurological conditions or spinal cord injuries can also affect the nerves that control erections.

3. Psychological Factors: Sometimes, psychological arousal can lead to prolonged erections, especially if there is significant sexual stimulation or anxiety involved.

4. Hormonal Imbalances: Hormonal issues, particularly involving testosterone, can also play a role in sexual function and erections.

If you are experiencing discomfort, persistent erection, or any other concerning symptoms, it is crucial to seek medical attention. A healthcare provider can evaluate your symptoms, review any medications you may be taking, and conduct necessary tests to rule out any serious underlying conditions.

In the meantime, here are some recommendations:
- Avoid Stimulation: If you find that certain activities or stimuli lead to prolonged erections, it may be wise to avoid them until you can consult with a healthcare professional.

- Stay Hydrated: Ensure you are drinking enough water, as dehydration can sometimes exacerbate discomfort.

- Monitor Symptoms: Keep track of any additional symptoms you may experience, such as pain, swelling, or changes in urination, and report these to your doctor.

- Consult a Specialist: If the issue persists or worsens, consider seeing a urologist who specializes in male reproductive health. They can provide a more detailed assessment and tailored treatment options.

In summary, while occasional prolonged erections can occur, especially in response to sexual arousal, persistent or painful erections should not be ignored. It is essential to seek medical advice to ensure that there are no underlying health issues and to receive appropriate treatment if necessary. Your health and well-being are paramount, and addressing these concerns with a healthcare professional is the best course of action.
