Abnormal bleeding

Hello Doctor, last week on 10/30 I inquired about abnormal bleeding.
You mentioned that I might be experiencing menstrual irregularities, but there was also a possibility of pregnancy, and advised me to wait and see if my period would come.
Well, my period did start that day, but it came after only 26 days, which is a bit of a difference from my usual cycle of 31-33 days.
Additionally, today is already the seventh day, and the flow has not decreased; it still feels like the same amount as on days three and four.
Could this be caused by menstrual irregularities? Is there still a possibility of pregnancy? Furthermore, I noticed a bulge in my lower right abdomen, and pressing firmly next to my hip bone causes soreness.
I wonder if this is related as well.
Thank you, Doctor.

Sophie, 20~29 year old female. Ask Date: 2016/11/05

Dr. Huang Yiwen reply Obstetrics and Gynecology

Hello Miss Sofie: Your current situation is also a type of menstrual disorder.
Common menstrual disorders include delayed menstruation or having multiple periods in a month, with varying durations.
As for the possibility of pregnancy, it is unlikely.
You can take a urine test to find out.
The pain when pressing on the protruding area in the lower right abdomen may not be related to the menstrual disorder.

Abnormal bleeding can be a significant concern for many women, and understanding its causes is essential for addressing any underlying health issues. Based on your description, it appears that you are experiencing a change in your menstrual cycle, which can be attributed to various factors, including hormonal imbalances, stress, lifestyle changes, and even potential pregnancy.

Firstly, it is important to note that menstrual cycles can vary significantly from month to month. A typical cycle ranges from 21 to 35 days, and variations can occur due to numerous reasons. In your case, you mentioned that your cycle has shifted from a typical range of 31-33 days to 26 days. This change, while concerning, is not uncommon and can be influenced by factors such as stress, changes in weight, exercise habits, or even changes in your daily routine.

The fact that your menstrual bleeding has persisted for seven days with a volume that has not decreased may indicate a few possibilities. One common cause of prolonged menstrual bleeding is a condition known as menorrhagia, which is characterized by heavy or prolonged menstrual bleeding. This can be caused by hormonal imbalances, uterine fibroids, polyps, or other underlying conditions. Given that you have already consulted with a healthcare provider, it is crucial to follow up if the bleeding continues or worsens, as further evaluation may be necessary.

Regarding the possibility of pregnancy, it is essential to consider that while you may have had your period, some women experience bleeding during early pregnancy, which can sometimes be mistaken for a menstrual period. If there is any chance of pregnancy, it would be advisable to take a home pregnancy test or consult with your healthcare provider for a blood test to confirm or rule out pregnancy.

The discomfort you are experiencing in your right lower abdomen could be related to several factors. It may be associated with normal menstrual cramping, but if the pain is significant or persistent, it could indicate other issues such as ovarian cysts, endometriosis, or even an ectopic pregnancy, especially if accompanied by abnormal bleeding. It is crucial to monitor this symptom closely and report it to your healthcare provider, particularly if it worsens or is accompanied by other symptoms such as fever, severe pain, or changes in bowel or urinary habits.

In summary, while your symptoms may be indicative of a hormonal imbalance or other benign conditions, it is essential to remain vigilant and proactive about your health. If your bleeding continues beyond the typical duration of a menstrual period, or if you experience significant pain or other concerning symptoms, I recommend scheduling a follow-up appointment with your healthcare provider. They may suggest further diagnostic tests, such as blood tests, ultrasounds, or even a pelvic exam, to determine the underlying cause of your symptoms and provide appropriate treatment options. Remember, your health is paramount, and seeking timely medical advice is always the best course of action.
