Postoperative penile curvature correction with tunica albuginea folding technique?

Dear Doctor,
I underwent penile curvature surgery with a tunica albuginea folding technique in November 2014, and it has been two years since then.
I still experience discomfort, swelling, pain, and numbness when standing.
During this time, I have been taking pain medication.
The original surgeon has said that it is normal and that there is no nerve damage, and an ultrasound of the penis showed normal results.
I have consulted other physicians at different medical centers, and one doctor suggested that there might be nerve damage.

My question is: could I have sustained nerve damage? Should I see a pain specialist for procedures like nerve blocks, or should I continue to wait?
Sincerely, a distressed patient.

Hello, it has been some time since the surgery, and there are many reasons for the ongoing discomfort.
Personally, I believe the chance of nerve injury is low; it may still be sensations caused by the surrounding tissue.
In addition to taking medication, I suggest trying vitamins, maintaining a regular exercise routine (such as walking for thirty minutes), showering instead of taking baths, and avoiding spicy or irritating foods for a few weeks.
I do not recommend nerve blocks.
Thank you for your inquiry.

Post-surgery discomfort after penile curvature correction, such as the procedure you underwent in November 2014, can be quite complex and multifaceted. It’s understandable that you are feeling frustrated and concerned about your ongoing symptoms, including pain, swelling, and numbness.
Firstly, it’s important to recognize that surgical interventions, especially those involving delicate structures like the penis, can lead to various complications and side effects. While many patients experience a smooth recovery, others may face persistent discomfort or complications that can last for months or even years after the surgery.
The symptoms you describe—pain, swelling, and numbness—could potentially indicate several issues. One possibility is nerve involvement. During surgery, there is a risk of inadvertently damaging nearby nerves, which can lead to sensations of pain, numbness, or altered sensation in the area. This is particularly relevant in procedures involving the penile region, where nerves are densely packed and play a crucial role in sensation and function.

Another consideration is the formation of scar tissue. After any surgical procedure, the body heals by forming scar tissue, which can sometimes lead to tightness or discomfort. If the scar tissue is extensive or improperly formed, it can cause pain or restrict movement, leading to the symptoms you are experiencing.

In terms of management, it is crucial to consult with a healthcare professional who specializes in pain management or urology. A pain specialist can provide a comprehensive evaluation of your symptoms and may recommend treatments such as nerve blocks, physical therapy, or other modalities to help alleviate your discomfort. Nerve blocks can be particularly effective if nerve damage is suspected, as they can help reduce pain by interrupting the pain signals sent to the brain.

It’s also worth considering that while waiting for symptoms to improve on their own is an option, it is not advisable to endure significant pain without seeking further evaluation. Persistent discomfort can impact your quality of life and may indicate an underlying issue that requires intervention.

In summary, while it is not uncommon to experience discomfort after penile curvature correction surgery, the persistence of your symptoms warrants further investigation. Consulting with a pain management specialist or a urologist who is experienced in post-surgical complications can provide you with a clearer understanding of your condition and potential treatment options. Don’t hesitate to seek help; addressing these concerns early can lead to better outcomes and improved quality of life.
