Erectile dysfunction? Premature ejaculation?

Hello, doctor.
How long is a normal duration for an erection? When erect, I can achieve about a 90-degree angle with my body, but when I stand up or stop stimulation, I feel a slight loss of firmness, similar to interrupting urination, and I need to contract my pelvic floor muscles and anus to maintain some firmness.
I used to masturbate quite frequently, about 1 to 3 times a day for 4 to 5 years, and I haven't exercised much.
Recently, I've been trying to quit masturbation, and now I only do it 1 to 2 times every 3 to 4 days.
I've also started doing squats and jogging.
Currently, my sexual experience is quite limited; during intercourse, I sometimes experience a slight loss of firmness after insertion.
When I use a normal position and go all out, I can last about a minute before ejaculation.
There was also an instance where, just as I was about to penetrate, the glans felt a bit tight, and I ejaculated right after penetration.
I can also achieve normal morning erections.
Could the issues mentioned above be due to a lack of sexual experience, psychological factors, or are they signs of erectile dysfunction or premature ejaculation?

It can be observed that adjustments can be gradually made with the accumulation of experience.

Erectile dysfunction (ED) and premature ejaculation (PE) are common concerns that affect many men at various stages of their lives. Understanding these conditions requires a nuanced approach, as they can be influenced by a variety of factors, including psychological, physiological, and lifestyle elements.

Understanding Normal Erectile Function
Firstly, it's important to clarify what constitutes a "normal" erection. An erection is generally considered normal if it can be achieved and maintained for sufficient time to engage in sexual activity. The duration of an erection can vary widely among individuals and situations, but it is not strictly defined by a specific time frame. Typically, an erection that lasts long enough for satisfactory sexual activity—often cited as anywhere from a few minutes to over 30 minutes—is considered normal. However, the quality of the erection, including firmness and the ability to maintain it during intercourse, is equally important.

Factors Influencing Erection Quality
In your case, you mentioned that your erection can reach about a 90-degree angle but feels less firm when standing or after stimulation stops. This could be related to several factors:
1. Psychological Factors: Anxiety, stress, and performance pressure can significantly affect erectile function. If you are feeling anxious about your sexual performance, this could lead to difficulties in maintaining an erection.

2. Physical Factors: The pelvic floor muscles, including the pubococcygeus (PC) muscle, play a crucial role in maintaining an erection. If these muscles are weak or not well-coordinated, it could lead to difficulties in sustaining an erection.

3. Lifestyle Choices: You mentioned a history of frequent masturbation and a lack of exercise, which can impact sexual health. While masturbation itself is not harmful, excessive frequency may lead to temporary desensitization or changes in sexual response. Incorporating regular exercise, such as squats and running, can improve blood flow and overall sexual health.

Premature Ejaculation Concerns
Regarding your experiences with ejaculation, it’s common for men, especially those with limited sexual experience, to encounter issues with premature ejaculation. The fact that you experience ejaculation shortly after penetration or during initial attempts can be attributed to several factors:
1. Inexperience: As you noted, your sexual experience is relatively limited. This can lead to heightened arousal and anxiety, which may contribute to premature ejaculation.

2. Physiological Responses: The tightness you feel during penetration could also be a factor. Sensitivity in the glans (the head of the penis) can lead to quicker ejaculation, especially if you are not accustomed to the sensations of penetration.

3. Psychological Factors: Similar to erectile function, anxiety about performance can lead to premature ejaculation. The pressure to perform can create a cycle of anxiety and premature ejaculation.

Recommendations
1. Communication: If you are engaging in sexual activity with a partner, open communication about your concerns can alleviate some pressure and enhance the experience for both of you.

2. Relaxation Techniques: Consider practicing relaxation techniques or mindfulness to help manage anxiety related to sexual performance.

3. Pelvic Floor Exercises: Strengthening your pelvic floor muscles through Kegel exercises can improve control over ejaculation and enhance erectile function.

4. Consult a Professional: If these issues persist or worsen, it may be beneficial to consult a healthcare professional or a urologist who specializes in sexual health. They can provide tailored advice and treatment options.

5. Limit Stimulation: Gradually reducing the frequency of masturbation may help restore sensitivity and improve your sexual response over time.

In conclusion, while your experiences may raise concerns about erectile dysfunction or premature ejaculation, they are not uncommon, especially among younger men with limited sexual experience. By addressing psychological factors, improving physical health, and seeking professional guidance if needed, you can work towards enhancing your sexual health and confidence.
