Light bleeding after sexual intercourse at 18 weeks of pregnancy?

Hello Doctor:
1.
My wife is 18 weeks pregnant.
After having intercourse today, she went to the bathroom and noticed a little blood on the toilet paper after wiping her vagina.
It only appeared a couple of times and then stopped, and she doesn’t feel any discomfort in her abdomen.
However, we are still very worried about her and the baby.

2.
Also, recently my wife occasionally feels a slight soreness in her vagina, but it goes away after a while.
Could this be due to not drinking enough water, or is it caused by something else?

Pu'en, 30~39 year old female. Ask Date: 2016/11/14

Dr. Yang Wangjun reply Obstetrics and Gynecology

Hello: (1) During pregnancy, the cervix becomes everted, and the columnar epithelium is more fragile, making the underlying microvessels prone to bleeding.
Typically, cervical bleeding does not affect the fetus.
However, if intercourse is too forceful, it may cause membrane rupture or placental abruption, which can impact the fetus and should be avoided.
(2) Around 4 to 5 months of pregnancy, noticeable uterine contractions may occur.
These contractions are usually painless, irregular in timing, and not intense, referred to as Braxton-Hicks contractions.
During these contractions, there may be a feeling of fullness in the lower abdomen and a sensation of discomfort in the vagina.
Thank you for your message!

Light bleeding during pregnancy, especially after sexual intercourse, can be a source of concern for many expectant parents. In your case, your wife is 18 weeks pregnant and has experienced light bleeding after sex, which is not uncommon. Here’s what you should know about this situation:
1. Light Bleeding After Sex: Light bleeding after sexual intercourse during pregnancy can occur for several reasons. One common cause is the increased blood flow to the pelvic area during pregnancy, which can make the vaginal tissues more sensitive and prone to bleeding. This is often referred to as "cervical bleeding," which can happen if the cervix is irritated during intercourse. As long as the bleeding is light and stops quickly, it is generally not a cause for alarm. However, it is crucial to monitor the situation. If the bleeding becomes heavier, is accompanied by pain, or if there are any other concerning symptoms, it is essential to contact a healthcare provider immediately.

2. Acidic Sensation in the Vagina: The occasional feeling of acidity or discomfort in the vaginal area can be attributed to various factors. It could be due to hormonal changes during pregnancy, which can affect the vaginal environment. Additionally, dehydration can lead to changes in vaginal secretions, potentially causing discomfort. It is important for your wife to stay well-hydrated, as adequate fluid intake is essential for overall health during pregnancy. If the sensation persists or worsens, it may be worth discussing with her healthcare provider to rule out any infections or other underlying issues.

3. When to Seek Medical Advice: While light bleeding after sex can be normal, it is always better to err on the side of caution. If your wife experiences any of the following, you should seek medical attention:
- Heavy bleeding (soaking through a pad in an hour or less)
- Severe abdominal pain or cramping
- Fever or chills
- Foul-smelling discharge
- Any other unusual symptoms that cause concern
4. General Advice for Sexual Activity During Pregnancy: Engaging in sexual activity during pregnancy is generally safe unless advised otherwise by a healthcare provider. It is important to communicate openly with your partner about comfort levels and any concerns. If your wife experiences discomfort during sex, it may be helpful to try different positions or use lubrication to reduce friction.

5. Regular Prenatal Care: Ensure that your wife continues to attend her regular prenatal check-ups. These visits are crucial for monitoring the health of both the mother and the baby. During these appointments, you can discuss any concerns, including bleeding and discomfort, with her healthcare provider.

In summary, while light bleeding after sex can be normal during pregnancy, it is essential to monitor the situation and seek medical advice if there are any concerning symptoms. Staying hydrated and maintaining open communication with your partner about any discomfort is also important. Regular prenatal care will help ensure a healthy pregnancy for both your wife and your baby.
