Breast Discharge and Skin Changes After Pregnancy Loss - Obstetrics and Gynecology

Nipple discharge, nipple peeling, mild itching that may occur?

Hello doctor, I had to undergo surgery on November 14 to remove a fetus (it was about 10 weeks along at that time).
On November 20, I noticed that when I pressed down on my breasts, a liquid (milky white and salty) would come out.
On November 22, I observed that the skin on my nipples was peeling (similar to sunburn peeling), and it is generally not itchy (though it is not completely itch-free).
Today, November 22, I still see bright red blood, and I'm unsure if this is normal (the amount is not significant).
Thank you for your response.

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, I apologize for not being able to diagnose based on the described symptoms.
It could be related to a benign condition.
If there is no improvement and discomfort persists, I recommend seeing a breast surgeon for evaluation.
Wishing you good health.

I'm sorry to hear about your recent loss, and it's understandable to have concerns about the changes you're experiencing in your body. After a pregnancy loss, it is common for women to experience various physical changes, including breast discharge and skin alterations.
Firstly, regarding the breast discharge you mentioned, it is not unusual for women to experience lactation or milk-like discharge after a pregnancy, even if the pregnancy ended early. This can happen due to hormonal changes in your body. During pregnancy, the hormone prolactin increases, which stimulates milk production. Even after a loss, it may take some time for hormone levels to return to their pre-pregnancy state, which can lead to the discharge you are observing. The discharge being white and salty is consistent with colostrum, which is the first form of milk produced by the breasts.

As for the peeling of the nipple skin, this can occur due to several reasons. Hormonal fluctuations can lead to changes in skin texture and sensitivity. The skin on the nipples may become dry or irritated, leading to peeling. This is often not a cause for concern, especially if it is not accompanied by significant itching or pain. However, if you notice any signs of infection, such as increased redness, swelling, or discharge that is not typical, it would be wise to consult a healthcare provider.

Regarding the fresh red blood you mentioned, it is important to monitor this closely. After a pregnancy loss, some bleeding is expected as your body expels the pregnancy tissue. However, if the bleeding is persistent, increasing in volume, or accompanied by severe pain, it is crucial to seek medical attention. Light spotting can be normal, but any significant changes should be evaluated by a healthcare professional.

In summary, the breast discharge and skin changes you are experiencing can be attributed to the hormonal adjustments following your pregnancy loss. It is essential to keep an eye on your symptoms and consult with a healthcare provider if you have any concerns, especially regarding the bleeding. They can provide you with the most accurate information and support tailored to your situation. Remember to take care of yourself during this time, both physically and emotionally, as recovery from a pregnancy loss can be a challenging process.
