Autonomic nervous system dysfunction

Yesterday, I went to the hospital to see a psychiatrist because I have anxiety disorder and autonomic nervous system dysregulation.
The doctor prescribed three medications: Oxazolam 10 mg, Fluoxetine, and Lorazepam.
After taking them for a day, I feel constantly fatigued.
Is this a side effect? How many days does it take for the antidepressant to take effect?

Dr. Liang Sunyuan reply Psychiatry

Hello: The side effect of fatigue typically diminishes gradually, and the effects of medication treatment usually become more noticeable within two to four weeks.

Understanding autonomic nervous system disorders and their treatment can be quite complex, especially when it comes to the medications prescribed for conditions like anxiety disorders. In your case, you have been prescribed three medications: Oxazolam, Fluoxetine, and Lorazepam. Each of these medications has its own mechanism of action, potential side effects, and timeline for effectiveness.

Oxazolam is a benzodiazepine that is primarily used to treat anxiety. It works by enhancing the effects of a neurotransmitter called gamma-aminobutyric acid (GABA), which has a calming effect on the brain. While it can provide quick relief from anxiety symptoms, it can also lead to side effects such as drowsiness, fatigue, and dizziness, especially when you first start taking it. These side effects are common with benzodiazepines and may diminish over time as your body adjusts to the medication.

Fluoxetine, on the other hand, is a selective serotonin reuptake inhibitor (SSRI) commonly prescribed for depression and anxiety disorders. It works by increasing the levels of serotonin in the brain, which can help improve mood and reduce anxiety. Unlike benzodiazepines, SSRIs like Fluoxetine typically take several weeks to show their full effects—usually around 4 to 6 weeks. During the initial period of treatment, some patients may experience side effects such as nausea, insomnia, or increased anxiety, but these often resolve as the body adjusts.

Lorazepam is another benzodiazepine that is often used for short-term relief of severe anxiety symptoms. Similar to Oxazolam, it can cause sedation and fatigue, especially when starting treatment. It is important to use Lorazepam cautiously, as prolonged use can lead to dependence.

Regarding your concern about feeling fatigued after taking these medications for just one day, it is not uncommon to experience such side effects initially. Fatigue can be a result of both Oxazolam and Lorazepam, as they are sedative medications. If this fatigue persists or worsens, it is essential to communicate this with your healthcare provider, as they may need to adjust your dosage or consider alternative treatments.

In terms of how long you should expect to wait for the medications to take effect, it varies. As mentioned, Fluoxetine may take several weeks to show significant improvement in symptoms, while Oxazolam and Lorazepam can provide more immediate relief but should be used with caution due to their potential for dependence.

It's also important to consider that managing autonomic nervous system disorders often requires a comprehensive approach that includes therapy, lifestyle changes, and possibly other medications. Cognitive-behavioral therapy (CBT) is particularly effective for anxiety disorders and can help you develop coping strategies to manage your symptoms.

In summary, while the medications you are taking can be effective in managing anxiety and autonomic nervous system disorders, it is crucial to monitor your response to them closely. Keep in touch with your healthcare provider about any side effects you experience, and be patient as your body adjusts to the medications. If you have ongoing concerns about fatigue or the effectiveness of your treatment, don't hesitate to reach out for further evaluation and support.
