Irregular menstrual periods and prolonged bleeding after abortion?

Hello doctor, my girlfriend took abortion pills at the obstetrics and gynecology department on October 10th.
Later, in early November, she had a normal menstrual period with regular bleeding.
However, a few days ago, the bleeding started normally but included dark brown blood.
After two or three days, the color of the blood has become increasingly dark, approaching black.
She has been under a lot of academic pressure, often staying up late to study and lacking sleep.
We visited the obstetrics and gynecology department again, and the doctor prescribed medication to stop the bleeding, but she felt very uncomfortable taking it and is reluctant to take medication.
The doctor is uncertain if this is her menstrual period, and a pregnancy test showed that she is not pregnant.
I am unsure if the doctor has any recommendations for how to care for her body.
I want to help her take care of her health but don't know where to start.
Thank you for your assistance.

In the case of a typical miscarriage, the blood loss is approximately two to three hundred milliliters, which most individuals can tolerate without causing anemia.
As you described, frequent late nights and insufficient sleep may be the primary cause of fatigue; therefore, it is advisable to rest well and ensure adequate sleep.
If the bleeding persists, in addition to hemostatic agents, you may also consider uterotonics or analgesics (which can also have a hemostatic effect).
Of course, if the aforementioned medications are ineffective, further medical attention is necessary.
Sincerely, Dr.

Managing irregular periods and heavy bleeding after an abortion can be a complex issue, especially when combined with factors such as stress, sleep deprivation, and the physical changes that follow a pregnancy termination. It’s important to approach this situation with care and understanding, as the body needs time to heal and regulate itself after such a significant event.

Firstly, it’s essential to recognize that after an abortion, whether medical or surgical, it is common for women to experience irregular bleeding and changes in their menstrual cycle. The body undergoes hormonal fluctuations as it adjusts to the absence of pregnancy, which can lead to variations in menstrual flow, color, and consistency. The presence of dark brown or even black blood can be alarming, but it often indicates older blood that is taking longer to exit the body. This is typically not a cause for concern unless accompanied by severe pain or other alarming symptoms.

Given that your girlfriend has experienced a mix of normal and dark brown bleeding, it’s crucial to monitor her symptoms closely. If the bleeding becomes excessively heavy (soaking through a pad in an hour or less), or if she experiences severe abdominal pain, fever, or any signs of infection, she should seek medical attention immediately. These could be signs of retained products of conception or other complications that may require further intervention.

Regarding her recent visit to the gynecologist and the prescription for hemostatic medication, it’s important to note that some women may experience side effects from these medications, leading to discomfort or aversion to taking them. If she feels strongly against taking the prescribed medication, it would be wise to communicate this to her healthcare provider. They might be able to suggest alternative treatments or supportive measures that could help manage her symptoms without causing discomfort.

In terms of lifestyle adjustments, there are several strategies that can help support her recovery and overall well-being:
1. Nutrition: Encourage her to maintain a balanced diet rich in iron, vitamins, and minerals. Foods such as leafy greens, lean meats, legumes, and nuts can help replenish nutrients lost during bleeding. Additionally, staying hydrated is crucial.

2. Rest and Sleep: Given her heavy academic workload, it’s vital to prioritize rest. Sleep deprivation can exacerbate stress and hormonal imbalances, potentially affecting her menstrual cycle. Encourage her to establish a regular sleep schedule and incorporate relaxation techniques, such as deep breathing or meditation, to help manage stress.

3. Gentle Exercise: While it’s important to avoid strenuous activities immediately after an abortion, gentle exercise like walking or yoga can promote circulation and overall well-being. However, she should listen to her body and avoid any activities that cause discomfort.

4. Follow-Up Care: Regular follow-up appointments with her healthcare provider are essential to monitor her recovery. If her menstrual irregularities persist beyond a few cycles, further evaluation may be necessary to rule out any underlying issues.

5. Emotional Support: The emotional impact of an abortion can be significant. Encourage her to talk about her feelings and consider seeking support from friends, family, or a mental health professional if she is struggling emotionally.

In conclusion, while irregular periods and heavy bleeding after an abortion can be concerning, they are often part of the body’s natural healing process. By focusing on nutrition, rest, gentle exercise, and maintaining open communication with her healthcare provider, your girlfriend can support her recovery. If symptoms persist or worsen, it’s crucial to seek further medical evaluation to ensure her health and well-being.
