Urination problems
Hello doctor, recently whenever I drink sugary beverages, I experience abdominal pain, or I have the same sensation when I wake up in the morning without drinking water.
The only way to relieve the pain is to drink water immediately, but it continues to occur the next day.
The characteristic is that when I don't drink water, my urine is dark yellow, but when I do drink water, my urine is completely clear, showing a significant contrast.
Additionally, I feel some resistance in my urethra during urination, making it not very smooth.
The most severe episode was this morning when I just woke up; I had intense abdominal pain and rushed to the bathroom to urinate, but the urine was very scant and dark yellow.
Around May or June of this year, I visited a nephrologist and urologist in China for a urine test, but the doctors said everything was normal and there were no issues.
I also had a kidney ultrasound, which also came back normal.
However, this problem has been bothering me for almost a year.
What should I do to resolve this?

Hello: First, drink plenty of water (only plain water, avoid beverages) totaling 2500cc-3000cc per day.
Eat more fruits and vegetables, and engage in moderate exercise for about half an hour daily.
If there is no improvement after 5 days, you should see a urologist again.

It sounds like you are experiencing a combination of urinary symptoms that are concerning and warrant further evaluation. Based on your description, you have noticed a significant change in your urine color, particularly a deep yellow when you are dehydrated and a clear, almost transparent color when you are well-hydrated. Additionally, you are experiencing abdominal pain after consuming sugary drinks and some difficulty with urination, including a sensation of resistance in the urethra.
First, let's address the color of your urine. Urine color can vary widely based on hydration levels, diet, and overall health. Dark yellow urine typically indicates dehydration, while clear urine suggests adequate hydration. The fact that your urine color changes so drastically based on your fluid intake is a normal physiological response. However, if you are consistently experiencing dark yellow urine despite drinking fluids, it may indicate that your body is not effectively processing or utilizing the fluids you are consuming.
The abdominal pain you experience after drinking sugary beverages could be related to several factors. Sugary drinks can sometimes irritate the bladder or lead to increased urine production, which might cause discomfort. Additionally, if you have any underlying gastrointestinal issues, such as gastritis or irritable bowel syndrome (IBS), these could also contribute to abdominal pain after consuming certain foods or drinks.
The sensation of resistance during urination and the pain you feel could indicate a potential issue with your urinary tract. This could be due to a urinary tract infection (UTI), bladder irritation, or even a stone in the urinary tract. The fact that you have had these symptoms for nearly a year, despite previous evaluations showing normal results, suggests that there may be a chronic issue that has not yet been identified.
Given that you have already undergone tests, including urinalysis and kidney ultrasound, and received normal results, it may be beneficial to seek a second opinion from a urologist or nephrologist. They may recommend further testing, such as a cystoscopy, which allows direct visualization of the bladder and urethra, or additional imaging studies to rule out any anatomical abnormalities.
In the meantime, consider keeping a detailed diary of your symptoms, including when they occur, what you eat and drink, and any other relevant factors. This information can be invaluable for your healthcare provider in diagnosing the issue.
Additionally, it may be helpful to reduce your intake of sugary beverages and see if that alleviates some of your symptoms. Staying well-hydrated with water is crucial, as it can help dilute your urine and potentially reduce irritation in the urinary tract.
In summary, while changes in urine color and abdominal pain can be concerning, they are often manageable with the right approach. It is essential to continue seeking medical advice and to advocate for your health until you find a resolution to your symptoms.
