Swelling between the testicles and the anus?
A month ago, there was swelling that improved with warm water soaks and massage.
It subsided, but yesterday during a bowel movement, I suddenly felt swelling again.
Normally, sitting in the office or moving around doesn't affect it; I only feel pain when I touch it.
Doctor, what could this condition be?

Dr. Du Shixi reply Urology
Hello: It could be an infection; please consult a urologist or a colorectal specialist (as the anus could also be a potential cause).
Thank you for your inquiry.

Swelling between the testicles and anus, often referred to as the perineal area, can be caused by a variety of conditions. Given your description of the swelling and the pain upon touch, it is essential to consider several potential causes, including but not limited to:
1. Inguinal Hernia: This occurs when tissue, such as part of the intestine, protrudes through a weak spot in the abdominal muscles. It can cause swelling and discomfort in the groin area, which may extend to the perineum.
2. Hydrocele: This is a fluid-filled sac surrounding a testicle that can lead to swelling in the scrotum. While hydroceles are generally painless, they can sometimes cause discomfort.
3. Epididymitis: This is the inflammation of the epididymis, the tube at the back of the testicle that stores and carries sperm. It can cause swelling and pain in the testicular area, which might radiate to the perineum.
4. Testicular Torsion: This is a medical emergency where the spermatic cord becomes twisted, cutting off blood supply to the testicle. It typically presents with severe pain and swelling and requires immediate medical attention.
5. Abscess or Infection: An abscess in the perineal area or an infection in the surrounding tissues can lead to localized swelling and tenderness. Conditions like prostatitis (inflammation of the prostate) can also cause similar symptoms.
6. Lymphadenopathy: Swelling of the lymph nodes in the groin area can occur due to infections or other systemic conditions, leading to discomfort and swelling in the perineal region.
7. Skin Conditions: Conditions such as folliculitis or other skin infections can also cause localized swelling and tenderness.
Given that you experienced swelling previously that resolved with heat and massage, it may suggest a transient condition like a mild infection or inflammation. However, the recurrence of swelling, especially if accompanied by pain, warrants further evaluation.
Recommendations:
1. Medical Evaluation: It is crucial to consult a healthcare professional for a thorough examination. They may perform a physical examination and possibly order imaging studies (like an ultrasound) to determine the underlying cause of the swelling.
2. Avoid Self-Diagnosis: While home remedies like warm baths and gentle massage may provide temporary relief, they should not replace professional medical advice.
3. Monitor Symptoms: Keep track of any additional symptoms, such as fever, changes in urination, or increased pain, and report these to your healthcare provider.
4. Avoid Strain: If you experience pain during bowel movements, consider dietary adjustments to prevent constipation, which can exacerbate discomfort in the perineal area.
5. Pain Management: Over-the-counter pain relievers may help alleviate discomfort, but consult with a healthcare provider before taking any medication, especially if you have underlying health conditions or are on other medications.
In summary, while there are several potential causes for swelling in the perineal area, a proper diagnosis from a healthcare professional is essential to determine the exact cause and appropriate treatment. Do not hesitate to seek medical attention, especially if the symptoms persist or worsen.
