Urinary issues
Doctor, recently I've been experiencing difficulty urinating.
I feel a tingling sensation in my glans and have a constant urge to urinate, but when I go to the bathroom, I can't seem to urinate at all.
It feels like there's a blockage.
However, when I do manage to urinate, it doesn't hurt.
Today, I tried to ejaculate spontaneously but couldn't, and I had to squat like I was having a bowel movement to squeeze out a little bit.
What could be the issue, doctor?

Hello: Please let me know if you are taking any medications.
If not, ensure adequate hydration (about 2500-3000 mL).
Avoid sugary drinks.
Make sure to get enough sleep, and try to walk for at least half an hour each day.
If your condition does not improve in 2-3 days, please seek medical attention.
Thank you for your inquiry.

More Info
Urination issues can be quite distressing and can stem from a variety of underlying causes. Based on your description, it seems you are experiencing a combination of urinary retention, urgency, and possibly some discomfort or unusual sensations in the genital area. Let’s break down the potential causes and solutions for your symptoms.
Possible Causes
1. Obstructive Uropathy: This condition occurs when there is a blockage in the urinary tract that prevents urine from flowing freely. This could be due to an enlarged prostate in men, urethral strictures, or even stones in the urinary tract. The sensation of feeling "blocked" and the difficulty in urinating could indicate some form of obstruction.
2. Neurogenic Bladder: If there is an issue with the nerves that control the bladder, it can lead to problems with urination. This can result in a feeling of urgency without the ability to urinate effectively. Conditions such as diabetes, multiple sclerosis, or spinal cord injuries can lead to neurogenic bladder.
3. Infection or Inflammation: Urinary tract infections (UTIs) or prostatitis (inflammation of the prostate) can cause symptoms such as urgency, frequency, and discomfort. While you mentioned there is no pain when urinating, infections can sometimes present with atypical symptoms.
4. Psychological Factors: Anxiety or stress can also manifest in physical symptoms related to urination. If you have a history of anxiety about using public restrooms or similar situations, this could contribute to your current symptoms.
5. Pelvic Floor Dysfunction: The muscles that control urination may not be functioning properly, leading to difficulty in starting urination or a sensation of incomplete emptying.
Recommended Solutions
1. Medical Evaluation: It is crucial to consult a urologist for a comprehensive evaluation. This may include a physical examination, urinalysis, ultrasound, or urodynamic studies to assess how well your bladder and urethra are functioning.
2. Medication: Depending on the diagnosis, medications may be prescribed. For example, if an enlarged prostate is the issue, alpha-blockers like tamsulosin can help relax the muscles around the prostate and bladder neck, making it easier to urinate.
3. Behavioral Techniques: If anxiety is a contributing factor, cognitive-behavioral therapy (CBT) or relaxation techniques may help alleviate some of the psychological barriers to urination.
4. Pelvic Floor Therapy: If pelvic floor dysfunction is suspected, working with a physical therapist who specializes in pelvic health can help strengthen and coordinate the muscles involved in urination.
5. Hydration and Diet: Ensure you are drinking enough fluids and consider dietary adjustments that may help reduce bladder irritants, such as caffeine and spicy foods.
6. Follow-Up: Regular follow-up with your healthcare provider is essential to monitor your symptoms and adjust treatment as necessary.
Conclusion
Your symptoms of difficulty urinating, the sensation of blockage, and the need to adopt certain positions to relieve your bladder are concerning and warrant further investigation. It is essential to address these issues promptly to prevent potential complications such as urinary retention or infections. Please seek medical attention as soon as possible to get a proper diagnosis and treatment plan tailored to your needs. Remember, you are not alone in this, and many effective treatments are available to help you regain normal urinary function.
