Pain after erection without intercourse?
Hello doctor, I have been experiencing a painful sensation since I started dating my girlfriend at the age of 17.
Every time I kiss or hug her, I get an erection.
However, after a period of time without intercourse, I experience severe pain in my lower right abdomen and right testicle.
Even a light touch to the testicle causes pain, to the point where I have to lie down and cannot move at all.
What could this condition be?
Mr. Gu, 20~29 year old female. Ask Date: 2016/12/25
Dr. Fu Shizhang reply Urology
It is recommended to visit a urology clinic to confirm whether there are any issues related to the testes or epididymis.
If examinations confirm that there are no problems and the pain is self-limiting, it may be physiologic pain triggered by an erection response.

More Info
Experiencing pain in the lower abdomen and testicles after an erection without intercourse can be distressing and uncomfortable. This condition may be related to several factors, including physiological responses, potential medical issues, and psychological components.
Possible Causes
1. Epididymal Hypertension (Blue Balls): This is a common term used to describe the discomfort that can occur when sexual arousal does not result in ejaculation. During arousal, blood flow to the genital area increases, leading to an erection. If ejaculation does not occur, the blood may remain in the area, causing a feeling of fullness and discomfort in the testicles and lower abdomen. This condition is usually temporary and resolves with ejaculation or time.
2. Testicular Torsion: Although less common, severe pain in the testicles can indicate a medical emergency known as testicular torsion, where the spermatic cord becomes twisted, cutting off blood supply to the testicle. This condition requires immediate medical attention to prevent permanent damage.
3. Inguinal Hernia: An inguinal hernia occurs when tissue, such as part of the intestine, protrudes through a weak spot in the abdominal muscles. This can cause pain in the lower abdomen and groin, especially during physical activity or straining.
4. Orchitis or Epididymitis: These are inflammatory conditions of the testicles (orchitis) or the epididymis (epididymitis), often caused by infections. Symptoms may include swelling, redness, and pain in the affected area.
5. Psychological Factors: Anxiety or stress related to sexual performance or experiences can also manifest as physical symptoms, including pain. The anticipation of sexual activity can lead to muscle tension and discomfort.
Solutions and Recommendations
1. Consult a Healthcare Provider: It is essential to consult a healthcare professional for a thorough evaluation. They may perform a physical examination and possibly imaging studies (like an ultrasound) to rule out serious conditions like testicular torsion or hernias.
2. Ejaculation: If the pain is related to epididymal hypertension, finding a way to relieve the pressure through ejaculation may help. This can be achieved through sexual activity or masturbation.
3. Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help alleviate discomfort. However, it is crucial to follow the recommended dosages and consult a doctor if the pain persists.
4. Avoid Strain: If you experience pain, avoid activities that may exacerbate the discomfort, such as heavy lifting or strenuous exercise.
5. Relaxation Techniques: Engaging in relaxation techniques such as deep breathing, meditation, or gentle stretching may help reduce anxiety and muscle tension, potentially alleviating some discomfort.
6. Follow-Up: If the pain continues or worsens, or if you experience additional symptoms such as swelling, fever, or changes in urination, seek medical attention promptly.
Conclusion
While experiencing pain in the lower abdomen and testicles after an erection without intercourse can be concerning, understanding the potential causes can help in addressing the issue. It is crucial to seek medical advice to rule out serious conditions and to receive appropriate treatment. Open communication with your healthcare provider about your symptoms and concerns is vital for effective management and relief.
